目的:探讨三维CAD颌骨模型在下颌骨肿瘤术前设计及下颌骨缺损个性化重建中的应用特点及临床效果。方法选取10例单侧下颌骨肿瘤患者,术前所有患者均行颌面部三维螺旋CT扫描,利用数据通过快速原型技术制作实体模型,参照模型设计截骨线并预弯重建钛板,手术按术前设计方案施行,切除下颌骨肿瘤并就位钛板。结果三维CAD颌骨模型与患者颌骨大小、形状、结构等十分接近,可以显示病变侵蚀范围,利用三维CAD颌骨模型进行术前设计,手术过程顺利,大大缩短了手术时间,术后效果满意。结论三维CAD颌骨模型可用于下颌骨肿瘤等术前设计,具有精度高、简便快捷等特点,具有可靠的应用价值。

목적:탐토삼유CAD합골모형재하합골종류술전설계급하합골결손개성화중건중적응용특점급림상효과。방법선취10례단측하합골종류환자,술전소유환자균행합면부삼유라선CT소묘,이용수거통과쾌속원형기술제작실체모형,삼조모형설계절골선병예만중건태판,수술안술전설계방안시행,절제하합골종류병취위태판。결과삼유CAD합골모형여환자합골대소、형상、결구등십분접근,가이현시병변침식범위,이용삼유CAD합골모형진행술전설계,수술과정순리,대대축단료수술시간,술후효과만의。결론삼유CAD합골모형가용우하합골종류등술전설계,구유정도고、간편쾌첩등특점,구유가고적응용개치。
Objective To explore the application features and clinical effects of preoperative design and individualized reconstruction of mandible with the three-dimensional CAD model of mandibular.Methods Ten patients with unilateral mandibular tumor,male 6 cases,fe-male 4 cases,aged 17 ~68,the average age of 41.5 years.All patients were scanned with 3D-CT before operation.Solid models and mirror models were made with rapid prototyping technology.According to the models,an osteotomy line was designed and individualized reconstructed Ti-palate pre-curved.After the resection of tumor,the individualized reconstructed Ti-palate was fixed in place to reconstruct the mandible. Results 3D models is very close to the mandible of the patients,and it can display the lesion area of damage on the model,it determine oste-otomy line and individualized reconstructed Ti-palate pre-curved,according to the preoperative design procedure,the procedure smoothly, shorten the operation time,postoperative effect is satisfactory.Conclusion The three-dimensional CAD model of mandibular can be used for oral maxillofacial deformities and jaw bone defect such as preoperative design,it has the characteristics of high precision,simpleand quick.It has reliable application value in mandibular tumor preoperative design.
