目的：评价降钙素原（PCT）联合白介素-6（IL-6）在重症肺炎中的诊断价值。方法收集89例社区获得性肺炎患者，分为重症肺炎组25例、普通肺炎组64例以及18例健康者为对照组。入院后24小时内采集PCT、IL-6标本并用免疫荧光法进行检测，同时记录各组患者的外周血血常规、红细胞沉降率、C反应蛋白（CRP）、细菌培养结果。结果重症肺炎组中PCT及IL-6值分别为（14．77±6．14）ng/mL、（271．8±57．95）mg/L，明显高于其他两组，差异有统计学意义（P＜0．05），普通肺炎组中两者均高于对照组，但差异无统计学意义。根据ROC曲线图，以PCT＞0．08 ng/mL为截断值，诊断细菌感染的敏感性为85．9％，特异性为67．6％；以IL-6＞14 mg/L为截断值，诊断细菌感染的敏感性为57％，特异性为83．8％。结论 PCT与IL-6在重症肺炎诊断中具有重要价值，同时检测二者有利于重症肺炎的诊治。
목적：평개강개소원（PCT）연합백개소-6（IL-6）재중증폐염중적진단개치。방법수집89례사구획득성폐염환자，분위중증폐염조25례、보통폐염조64례이급18례건강자위대조조。입원후24소시내채집PCT、IL-6표본병용면역형광법진행검측，동시기록각조환자적외주혈혈상규、홍세포침강솔、C반응단백（CRP）、세균배양결과。결과중증폐염조중PCT급IL-6치분별위（14．77±6．14）ng/mL、（271．8±57．95）mg/L，명현고우기타량조，차이유통계학의의（P＜0．05），보통폐염조중량자균고우대조조，단차이무통계학의의。근거ROC곡선도，이PCT＞0．08 ng/mL위절단치，진단세균감염적민감성위85．9％，특이성위67．6％；이IL-6＞14 mg/L위절단치，진단세균감염적민감성위57％，특이성위83．8％。결론 PCT여IL-6재중증폐염진단중구유중요개치，동시검측이자유리우중증폐염적진치。
Objective To evaluate the diagnosis of serum procalcitonin (PCT)and inter-leukin-6 (IL-6 )in severe pneumonia. Methods 89 patients who underwent community acquired pneumonia in our hospital from Jan.2012 to Sep.2013 were divided into severe pneumonia group with 25 patients and mild pneumonia group with 64 patients,and compared with the 18 healthy people.during the first 24 hours who were hospitalized,the specimens were collected,PCT and IL-6 levels were determined using Fluorescence immunoassay regularly,at the same time,the following parameters were assessed,such as white blood cell count,erythrocyte sedimentation rate(ESR),C-reactive protein (CRP)and bacterial culture of both sputum and blood.Results The Mean ±SD of PCT and IL-6,respectively were(14.77 ±6.14)mg/L and(271.8 ±57.95)mg/L in severe pneumonia group,which was significantly higher than that in mild pneumonia group and control group (P<0.05),the Mean ±SD of PCT and IL-6 in mild pneumonia group were also higher than that in control group,but the P-value >0.05. Depending on Receiver operator characteristic(ROC)curves,at a cut-off value,0.08 ng/mL PCT,the sensitivity was 85.9%,the specificity was 67.6%,at a cut-off value,14 mg/L IL-6,the sensitivity was 57%,the specificity was 83.8%.Conclusion Serum PCT and IL-6 play an important diagnosis value in severe pneumonia,at the same time detection,which is helpful for the diagnosis and treatment of severe pneumonia.
