目的:探讨超声二维斑点追踪技术(2D-STI)定量评价缩窄性心包炎( CP)患者左室纵向运动功能的价值。方法 CP患者40例( CP组),健康志愿者40例(对照组),应用2 D-STI技术测量两组观察对象心尖3个长轴(四腔心切面、二腔心切面及左心室长轴切面)总体纵向应变值及左心室各个节段纵向应变值。结果 CP组患者心尖3个长轴(四腔心切面、二腔心切面及左心室长轴切面)总体纵向应变值明显低于对照组( P<0.05);CP组患者有心包覆盖区域(侧壁、前壁、后室间隔基底段、后室间隔心尖段、下壁中间段、下壁心尖段、后壁中间段、后壁心尖段、前间隔心尖段及前间隔中间段)纵向应变值明显低于对照组(P<0.05),无心包覆盖区域(后室间隔中间段、下壁基底段及前间隔基底段)纵向应变值与对照组比较,差异无统计学意义(P>0.05)。结论 CP患者心脏总体纵向运动降低,病变早期有心包覆盖区域心肌存在损害;2 D-STI技术能定量评估CP患者心肌功能,反映心肌损伤情况。

목적:탐토초성이유반점추종기술(2D-STI)정량평개축착성심포염( CP)환자좌실종향운동공능적개치。방법 CP환자40례( CP조),건강지원자40례(대조조),응용2 D-STI기술측량량조관찰대상심첨3개장축(사강심절면、이강심절면급좌심실장축절면)총체종향응변치급좌심실각개절단종향응변치。결과 CP조환자심첨3개장축(사강심절면、이강심절면급좌심실장축절면)총체종향응변치명현저우대조조( P<0.05);CP조환자유심포복개구역(측벽、전벽、후실간격기저단、후실간격심첨단、하벽중간단、하벽심첨단、후벽중간단、후벽심첨단、전간격심첨단급전간격중간단)종향응변치명현저우대조조(P<0.05),무심포복개구역(후실간격중간단、하벽기저단급전간격기저단)종향응변치여대조조비교,차이무통계학의의(P>0.05)。결론 CP환자심장총체종향운동강저,병변조기유심포복개구역심기존재손해;2 D-STI기술능정량평고CP환자심기공능,반영심기손상정황。
Objective To investigate the clinical value of two-dimensional speckle tracking imaging (2D-STI) in evaluation on left ventricular longitudinal movement function in patients with constrictive pericarditis (CP).Methods Forty patients with CP(CP group) and 40 healthy volunteers(control group) were measured with 2D-STI for left ventricle overall longitudinal strain and per section longitudinal strain in three sections ( four-chamber view ,two-chamber view and left ventricular long-axis view ) .Results The three longitudinal strains of CP group were significantly lower than those of control group ( P<0 .05 ) .The left ventricle section longitudinal strains with pericardium covered area of CP group were significantly lower than those of control group ( P<0 .05 ) .Those without pericardium covered area showed no significant difference between CP group and control group ( P>0 .05 ) .Conclusion Left ventricle overall longitudinal strain reduces in CP patients .Myocardial damage exists in pericardium covered area of early stage CP .2 D-STI can be applied to quantitatively evaluating myocardial function as well as reflecting characteristic of myocardial damage in CP patients .
