目的:探讨门控心肌灌注显像对儿童病毒性心肌炎的诊断及预后价值。方法应用99锝m -甲氧基异丁基异腈对36例病毒性心肌炎患儿进行门控心肌灌注显像,观察心肌血流灌注情况,了解左心室收缩和舒张功能等心功能变化。结果血流灌注异常33例(91.7%),以多病灶损害为主;常见损害部位为前壁及下后壁。心功能异常19例(52.8%),其中,左心室射血分数降低8例,高峰充盈率降低13例,相程角增宽15例。结论门控心肌灌注显像是一种无创性检查方法,能直观显示心肌病变的部位、范围和严重程度,获得心脏功能参数,对病毒性心肌炎诊断及预后判断有重要价值。

목적:탐토문공심기관주현상대인동병독성심기염적진단급예후개치。방법응용99득m -갑양기이정기이정대36례병독성심기염환인진행문공심기관주현상,관찰심기혈류관주정황,료해좌심실수축화서장공능등심공능변화。결과혈류관주이상33례(91.7%),이다병조손해위주;상견손해부위위전벽급하후벽。심공능이상19례(52.8%),기중,좌심실사혈분수강저8례,고봉충영솔강저13례,상정각증관15례。결론문공심기관주현상시일충무창성검사방법,능직관현시심기병변적부위、범위화엄중정도,획득심장공능삼수,대병독성심기염진단급예후판단유중요개치。
Objective To investigate the clinical diagnostic and prognostic value of gated myocardial perfusion imaging in children with viral myocarditis (VMC).Methods 99m Tc-MIBI was used to perform gated myocardial perfusion imaging in 36 children with VMC to observe the characteristics of myocardial blood perfusion and the changes in left ventricular systolic and diastolic function .Results There were 33 cases(91.7%) of abnormal perfusion ,dominated by multi-focus injury ,commonly observed in the front wall and rear wall of the heart .Among the 19 cases ( 52.8%) of abnormal cardiac function,8 of decreased left ventricular ejection fraction ( LVEF),13 of decreased peak filling rate ( PFR) and 15 of increased phase shift .Conclusion The gated myocardial perfusion imaging is a noninvasive method , which could intuitionally reveal the location ,range and severity of myocardial lesions and obtain the cardiac function parameters .It is valuable for the diagnosis and prognosis of VMC .
