目的：了解ICU机械通气患者再插管率及再插管原因。方法分析厦门大学附属中山医院急诊重症监护室（EICU）2011年1月至2013年11月期间住院气管插管机械通气患者，经历一次及以上气管拔管患者。结果本研究再插管发生率22．33％，再插管同患者的基础疾病分类及拔管前的营养状况相关。结论再插管患者预后明显不佳，有较高的死亡率和气管切开率。应尽量避免意外拔管，对高龄患者拔管前应行计划性呼吸肌功能锻炼。
목적：료해ICU궤계통기환자재삽관솔급재삽관원인。방법분석하문대학부속중산의원급진중증감호실（EICU）2011년1월지2013년11월기간주원기관삽관궤계통기환자，경력일차급이상기관발관환자。결과본연구재삽관발생솔22．33％，재삽관동환자적기출질병분류급발관전적영양상황상관。결론재삽관환자예후명현불가，유교고적사망솔화기관절개솔。응진량피면의외발관，대고령환자발관전응행계화성호흡기공능단련。
Objective To know the rate of reintubation of mechanical ventilation patients in ICU and analysis the possible reason of reintubation .Methods Mechanical ventilation patients were in EICU from Jan 2011 to Nov 2013,who passed once or more intubation .Results Rate of reintubation was 22.33%,reintubation was related with the basis of disease classification and nutritional status before extubation .Conclusions Prognosis of reintubation patients is worse .Reintubation patients had higher mortality rate and tracheotomy rate . We should avoid accidental extubation and inspiratory muscle strength training before extubationis necessary for advanced age patient .
