目的：评价甘露聚糖肽联合顺铂胸腔内灌注治疗恶性胸腔积液的临床疗效及毒副反应。方法：60例恶性胸腔积液患者随机分为两组。所有患者均在经中心静脉导管胸腔持续闭式引流后灌注治疗，治疗组（31例）：甘露聚糖肽80-100mg，顺铂60mg 胸腔内灌注；对照组（29例）：单用顺铂60mg 胸腔内灌注。上述治疗每周2次，连续1-2周。结果：治疗组总有效率为83.9%（26/31），明显优于对照组的58.6%（17/29），差异有显著意义（P ﹤0.05）。治疗组生活质量改善率为71.0%，也显著高于对照组的44.8%（P ﹤0.05）。毒副反应主要为轻度消化道反应、白细胞降低及发热、胸痛。结论：甘露聚糖肽联合顺铂治疗恶性胸腔积液有良好的协同作用，能改善生活质量，毒副反应轻，值得临床广泛应用。

목적：평개감로취당태연합순박흉강내관주치료악성흉강적액적림상료효급독부반응。방법：60례악성흉강적액환자수궤분위량조。소유환자균재경중심정맥도관흉강지속폐식인류후관주치료，치료조（31례）：감로취당태80-100mg，순박60mg 흉강내관주；대조조（29례）：단용순박60mg 흉강내관주。상술치료매주2차，련속1-2주。결과：치료조총유효솔위83.9%（26/31），명현우우대조조적58.6%（17/29），차이유현저의의（P ﹤0.05）。치료조생활질량개선솔위71.0%，야현저고우대조조적44.8%（P ﹤0.05）。독부반응주요위경도소화도반응、백세포강저급발열、흉통。결론：감로취당태연합순박치료악성흉강적액유량호적협동작용，능개선생활질량，독부반응경，치득림상엄범응용。
To evaluate the clinical efficacy and adverse reactions of thoracic cavity perfusion with man-natide plus cisplatin(DDP)in treatment of malignant pleural effusion. Methods:Total of 60 patients with malignant pleural effusion were randomly divided into two groups. All patients underwent thoracic closed drainage continuously via central venous catheter. When pleural effusion disappear,31 cases in treatment group were treated with mannatide (80 - 100mg)plus DDP(60mg)by intra - pleural injection;29 cases in control group only received DDP 60mg,The treatment for both groups was repeated twice per week and continued for 1 - 2 weeks. Results:The response rate of the treatment group was 83. 9%(26 / 31),obviously higher than 58. 6%(17 / 29)of the control group. There was statisti-cally significant difference between the two groups(P ﹤ 0. 05). Life quality improvement rate of the treatment group was 71. 0% ,higher than 44. 8% of the control group(P ﹤ 0. 05). The main side - effect of two groups were mild gas-trointestinal reaction,eukopenia,fever and chest pain. Conclusion:Intra - pleural injection of mannatide plus DDP to treat of malignant pleural effusion can produce synergetic action,to be more effective and benefit to improve the pa-tient's life quality,and adverse reactions are mild.
