目的:探讨网织红细胞血红蛋白含量(reticulocyte hemoglobin content ,CHr)检测在筛查女性隐性铁缺乏中的应用价值。方法选取健康体检中心女性体检者为研究对象:证实其中69例为隐性铁缺乏组,334例为健康对照。检测所有入选者的CHr、红细胞计数(RBC)、血红蛋白(HGB)、血细胞比容(HCT)、平均红细胞体积(MCV)、平均红细胞血红蛋白(MCH)、平均红细胞血红蛋白浓度(MCHC)、红细胞分布宽度(RDW)、血清铁蛋白(SF)、血清铁(SI)、转铁蛋白(TRF)及总铁结合力(TIBC)等指标。结果隐性铁缺乏组CHr[(28.7±1.8)pg]明显低于健康对照组[(30.6±1.2)pg],差异具有显著的统计学意义(P<0.001)。ROC曲线证实诊断女性铁缺乏CHr的曲线下面积为0.876,明显高于RBC、HGB、HCT、MCV、MCH、MCHC、RDW等指标,差异具有统计学意义(均P<0.05)。结论 CHr可以用于筛查女性的隐性铁缺乏。

목적:탐토망직홍세포혈홍단백함량(reticulocyte hemoglobin content ,CHr)검측재사사녀성은성철결핍중적응용개치。방법선취건강체검중심녀성체검자위연구대상:증실기중69례위은성철결핍조,334례위건강대조。검측소유입선자적CHr、홍세포계수(RBC)、혈홍단백(HGB)、혈세포비용(HCT)、평균홍세포체적(MCV)、평균홍세포혈홍단백(MCH)、평균홍세포혈홍단백농도(MCHC)、홍세포분포관도(RDW)、혈청철단백(SF)、혈청철(SI)、전철단백(TRF)급총철결합력(TIBC)등지표。결과은성철결핍조CHr[(28.7±1.8)pg]명현저우건강대조조[(30.6±1.2)pg],차이구유현저적통계학의의(P<0.001)。ROC곡선증실진단녀성철결핍CHr적곡선하면적위0.876,명현고우RBC、HGB、HCT、MCV、MCH、MCHC、RDW등지표,차이구유통계학의의(균P<0.05)。결론 CHr가이용우사사녀성적은성철결핍。
Objective To examine the value of the reticulocyte hemoglobin content (CHr) test in screening for hidden iron deficiency in women. Methods Healthy female subjects in the center of medical examination were selected, and 69 cases were proved to be with hidden iron deficiency, and 334 cases were healthy controls. The levels of CHr, red blood cell counting (RBC), hemoglobin(HGB), hematocrit(HCT), mean corpuscular volume (MCV), mean corpuscular hemoglobin(MCH), mean corpuscular hemoglobin concentration(MCHC), red blood cell volume distribution width(RDW), serum ferritin (SF), serum iron(SI), transferrin(TRF), and total iron binding capacity(TIBC) were determined. Results The levels of CHr in the hidden iron deficiency group [(28.7±1.8)pg] were significantly lower than that in the control group [(30.6±1.2)pg] (P<0.001). Receiver operator characteristic curve analysis demonstrated that the area under the curve of CHr was 0.876 and significantly greater than RBC, HGB, HCT, MCV, MCH, MCHC and RDW in the diagnosis of hidden iron deficiency in women(all P<0.05). Conclusion CHr could be used for screening for hidden iron deficiency in women.
