目的:探讨双源CT冠状动脉成像对冠状动脉变异的检出率及诊断价值。方法回顾性分析5087例冠状动脉CT血管成像资料,通过容积再现、最大密度投影、曲面重组及多平面重组等多种后处理技术方法进行重建,对冠状动脉走向及变异进行分析。结果5087例冠状动脉血管成像中共检出冠状动脉变异139例,包括:冠状动脉起源异常98例,检出率1.93%,其中高位开口35例,起自对侧冠状窦或对侧冠状动脉25例,起自无冠窦3例,左主干缺如(双开口)4例,单支冠状动脉4例,副冠状动脉27例;冠脉发育不良32例,检出率0.63%,其中右冠发育不良12例,左冠发育不良20例;冠状动脉瘘9例,检出率0.18%,其中冠状动脉右室瘘4例,冠状动脉左室瘘2例,冠状动脉肺动脉瘘3例。本研究不包括心机桥及窦房结动脉变异。结论双源CT冠状动脉成像可准确评价冠状动脉正常及变异细节,是检出冠状动脉变异既安全有效又准确的方法。

목적:탐토쌍원CT관상동맥성상대관상동맥변이적검출솔급진단개치。방법회고성분석5087례관상동맥CT혈관성상자료,통과용적재현、최대밀도투영、곡면중조급다평면중조등다충후처리기술방법진행중건,대관상동맥주향급변이진행분석。결과5087례관상동맥혈관성상중공검출관상동맥변이139례,포괄:관상동맥기원이상98례,검출솔1.93%,기중고위개구35례,기자대측관상두혹대측관상동맥25례,기자무관두3례,좌주간결여(쌍개구)4례,단지관상동맥4례,부관상동맥27례;관맥발육불량32례,검출솔0.63%,기중우관발육불량12례,좌관발육불량20례;관상동맥루9례,검출솔0.18%,기중관상동맥우실루4례,관상동맥좌실루2례,관상동맥폐동맥루3례。본연구불포괄심궤교급두방결동맥변이。결론쌍원CT관상동맥성상가준학평개관상동맥정상급변이세절,시검출관상동맥변이기안전유효우준학적방법。
Objective To study of the diagnosis value of dual-source CT (DSCT)angiography for congenital coronary artery anomalies (CCAA). Methods We retrospectively analysised the date of 5087 patients by DSCT angiography. Congenital coronary artery anomalies were shown and analysised by volume rendering (VR), maxmium intensity projection(MIP),curved planar reconstruction (CPR), multi-planar reconstruction (MPR) and other post-processing techniques. Results 139 cases of congenital coronary artery anomalies were found in 5087 patients, including 98 cases of anomalous origin of the coronary artery (1.93%) (which included 35 cases of coronary artery with high origin, 25 cases of coronary artery dated from contralateral aortic sinus or contralateral coronary artery, 3 cases of coronary artery dated from posterior aorticsinus, 4 cases of absence of left main trunk,4 cases of single coronary artery, 27 cases of para-coronary artery), 32 cases of coronary artery dysplasia (0.63%),9 cases coronary artery fistula (0.18%).This study did not included myocardial bridge and the sinus node artery anomalies. Conclusion Dual-source CT coronary artery angiography is a safe ef ective and accurate in finding of congenital coronary artery anomalies.
