目的:了解抑郁症患者糖脂代谢以及应激情况。方法68例抑郁症患者作为抑郁症组,分别采用汉密顿抑郁量表(HAMD17)和汉密顿焦虑量表(HAMA14)、生活事件量表(LES)对患者情绪状况以及应激状况进行评定,监测其空腹血糖(FBG)、甘油三脂(TG)、血清总胆固醇(TC)、皮质醇(CO)水平,与40例对照组进行比较。结果抑郁症组的FBG、TG、CO水平显著高于对照组(P<0.01);而TC水平两组之间差异无统计学意义(P>0.05);同时相关分析发现,抑郁症患者LES总分与FBG、CO水平呈正相关(P<0.05或P<0.01);抑郁症患者的HAMA总分、精神焦虑因子分及躯体焦虑因子分与TG水平呈正相关。结论抑郁症患者糖脂代谢存在异常,与患者长期的精神刺激有关。所以,临床医师应早期监测抑郁症患者的血糖、血脂及皮质醇水平,以便早发现早治疗。

목적:료해억욱증환자당지대사이급응격정황。방법68례억욱증환자작위억욱증조,분별채용한밀돈억욱량표(HAMD17)화한밀돈초필량표(HAMA14)、생활사건량표(LES)대환자정서상황이급응격상황진행평정,감측기공복혈당(FBG)、감유삼지(TG)、혈청총담고순(TC)、피질순(CO)수평,여40례대조조진행비교。결과억욱증조적FBG、TG、CO수평현저고우대조조(P<0.01);이TC수평량조지간차이무통계학의의(P>0.05);동시상관분석발현,억욱증환자LES총분여FBG、CO수평정정상관(P<0.05혹P<0.01);억욱증환자적HAMA총분、정신초필인자분급구체초필인자분여TG수평정정상관。결론억욱증환자당지대사존재이상,여환자장기적정신자격유관。소이,림상의사응조기감측억욱증환자적혈당、혈지급피질순수평,이편조발현조치료。
Objective To study the situation of glucolipid metabolism and stress in patients with depression. Methods A total of 68 depression patients were taken as the depression group, and Hamilton depression scale (HAMD17), Hamilton anxiety scale (HAMA14) and life events scale (LES) were used to assess their mood and stress situation. Comparisons were made between the depression group and the control group with 40 cases in the monitoring results of their fasting blood glucose (FBG), triglyceride (TG), total cholesterol (TC), and cortisol (CO). Results The levels of FBG, TG and CO in the depression group were higher than those of the control group (P<0.01), while the difference of TC between the two groups was not statistically significant (P>0.05). The results of related analysis showed that LES score of depression patients was positively correlated with levels of FBG and CO (P<0.05 or P<0.01), and the HAMA score, mental anxiety factors score and physical anxiety factors score were all positively correlated with level of TG. Conclusion The abnormal glucolipid metabolism of depression patients is related with their long-term mental stimulation. Therefore, clinicians ought to take early monitor on the patients’blood glucose, blood lipid and cortisol so as to provide timely treatment.
