目的:检测骨原发性关节炎(OA)患者骨桥蛋白(OPN)、IL-1β表达水平的变化及意义,并探讨其与骨关节炎的相关性。方法选取2012年1月-2014年1月确诊为原发性 OA 患者的软骨标本20例作为 OA 组,以同期因意外受伤截肢的20例正常软骨标本为对照组。采用 HE 染色法观察组织学变化,免疫组化法检测关节软骨中OPN、IL-1β表达水平,并采用 SamplePCI 图象分析系统进行半定量分析。结果正常软骨细胞外基质染色均匀,分散于软骨基质之中,细胞核着色清晰均匀,胞核、胞质清晰可辨。随着 OA 病变程度的加重软骨细胞基质染色不均,甚至出现纤维化,细胞数目逐渐减少。经免疫染色后,OPN、IL-1β吸光值表示 OPN、IL-1β表达水平均升高(P ﹤0.05)。相关分析得出,OPN 与 IL-1β呈正相关(r = O.899,P ﹤0.01)。软骨病变程度分级与 OPN、IL-1β呈正相关(r =0.909,0.904,P 均﹤0.01)。结论原发性 OA 患者软骨组织中 OPN、IL-1β表达水平升高,且与病变程度具有密切联系,为OA 治疗的靶点提供了依据。

목적:검측골원발성관절염(OA)환자골교단백(OPN)、IL-1β표체수평적변화급의의,병탐토기여골관절염적상관성。방법선취2012년1월-2014년1월학진위원발성 OA 환자적연골표본20례작위 OA 조,이동기인의외수상절지적20례정상연골표본위대조조。채용 HE 염색법관찰조직학변화,면역조화법검측관절연골중OPN、IL-1β표체수평,병채용 SamplePCI 도상분석계통진행반정량분석。결과정상연골세포외기질염색균균,분산우연골기질지중,세포핵착색청석균균,포핵、포질청석가변。수착 OA 병변정도적가중연골세포기질염색불균,심지출현섬유화,세포수목축점감소。경면역염색후,OPN、IL-1β흡광치표시 OPN、IL-1β표체수평균승고(P ﹤0.05)。상관분석득출,OPN 여 IL-1β정정상관(r = O.899,P ﹤0.01)。연골병변정도분급여 OPN、IL-1β정정상관(r =0.909,0.904,P 균﹤0.01)。결론원발성 OA 환자연골조직중 OPN、IL-1β표체수평승고,차여병변정도구유밀절련계,위OA 치료적파점제공료의거。
Objective detection of bone in patients with primary osteoarthritis(OA)osteopontin(OPN),IL-1 beta change and significance of the expression level,and explore its correlation with osteoarthritis. Methods Between January 2012 and January 2012 diagnosed with primary cartilage specimens of 20 patients with OA as OA group,with the same period,ampu-tation because of accidental injury,20 specimens of normal cartilage as control group. Using HE staining to observe the histolog-ical changes,immunohistochemical method in the detection of articular cartilage OPN expression level,IL-1 beta,and semi-quantitative analysis using SamplePCI image analysis system. Results Normal cartilage extracellular matrix dyeing uniformity, scattered in cartilage matrix,uniform color clear,nucleus nuclei and clear cytoplasm. As the degree of OA lesions increase car-tilage matrix uneven dyeing,appear even fibrosis,the number of cells reduce gradually. After immune dyeing,OPN,IL-1 beta absorbance value indicates that OPN,IL-1 beta expression levels were elevated(P ﹤ 0. 05). Correlation analysis,OPN was positively correlated with IL-1 beta(r = 0. 899,P ﹤ 0. 01). Cartilage lesions in the classification and OPN,IL-1 beta were posi-tively correlated(r = 0. 909,0. 904,P ﹤ 0. 01). Conclusion OPN in patients with primary OA cartilage,IL-1 beta expression levels,and the degree is closely related with pathological changes,provide the basis for OA treatment targets.
