背景:多孔钽金属棒的研制与临床应用,为行保留股骨头手术提供了新的思路和方法。目前对于该术式的早期疗效比较肯定,尚缺乏中远期的随访。目的:探讨应用钽棒治疗早期股骨头坏死的中远期临床疗效。方法:2008年5月至2011年5月运用钽棒治疗早期股骨头坏死患者65例80髋,评估手术前、术后3个月及末次随访的Harris评分,末次随访的股骨头生存率。结果:全部获得随访,随访时间为36~60个月,平均48个月。股骨头生存率可达90%,其中SteinbergⅠ期生存率100%,Ⅱ期生存率95%,ⅢA期生存率73%。8例术后6~8个月出现股骨头塌陷,其余各髋未见进一步塌陷,末次随访X线片示假体位置良好。结论:钽棒植入术是治疗早期股骨头坏死的有效手段,其中远期疗效满意。

배경:다공단금속봉적연제여림상응용,위행보류고골두수술제공료신적사로화방법。목전대우해술식적조기료효비교긍정,상결핍중원기적수방。목적:탐토응용단봉치료조기고골두배사적중원기림상료효。방법:2008년5월지2011년5월운용단봉치료조기고골두배사환자65례80관,평고수술전、술후3개월급말차수방적Harris평분,말차수방적고골두생존솔。결과:전부획득수방,수방시간위36~60개월,평균48개월。고골두생존솔가체90%,기중SteinbergⅠ기생존솔100%,Ⅱ기생존솔95%,ⅢA기생존솔73%。8례술후6~8개월출현고골두탑함,기여각관미견진일보탑함,말차수방X선편시가체위치량호。결론:단봉식입술시치료조기고골두배사적유효수단,기중원기료효만의。
Background: Great development and clinical application of tantalum provides a new train of thought and method for re-served operations of femoral head. Early certain curative effects have been reported, but the surgical procedure still lacks of long-term follow-up. Objective:To explore the long-term curative effect of tantalum rod for the treatment of early avascular necrosis of femoral head. Methods:From May 2008 to May 2011, 65 patients (80 hips) with early femoral head necrosis were treated by tantalum rod implantation. Harris score was recorded before operation, 3 months after operation and at the last follow-up. The survival rate of femoral head was evaluated 36-60 months after surgery. Results:The mean duration of follow-up was 48 months (range, 36-60 months), and the survival rate of femoral head was 90%. The SteinbergⅠperiod survival rate was 100%,Ⅱperiod was 95%, andⅢA period was 73%. Femoral head col-lapsed in 8 cases 6 to 8 months postoperatively. No further collapse occurred in the rest of the hips, and X ray results showed the prosthesis was in good position at the last follow-up. Conclusions: Tantalum rod implantation is an effective method for treatment of early avascular necrosis of femoral head. The long-term follow-up results are encouraging.
