目的:观察不同分期糖尿病肾病(DN)患者血清游离脂肪酸(FFA)、同型半胱氨酸(Hcy)、胱抑素 C(CysC)等水平变化及与尿微量白蛋白排泄率(UAER)的相关性。方法:2型糖尿病(T2DM)患者178例,根据24h UAER分为单纯T2DM组(DM组,n=82)、早期DN组(EDN,n=56);临床DN组(CDN,n=40例);同期体检健康者作为对照组(n=42)。采用全自动生化分析仪测定各组血清 FFA(酶法)、Hcy(循环酶法)、CysC(免疫增强比浊法)、尿素氮(Urea,酶法)、肌酐(Cr,酶法)含量,采用全自动特种蛋白分析仪测定 UAER,比较各组各指标水平差异,分析EDN、CDN组患者 FFA、Hcy、CysC与 UAER的相关性。结果:EDN 组和 CDN 组血清 FFA、Hcy、CysC 水平高于DM组和对照组(P均<0.05),且CDN组高于 EDN组,DM组高于对照组(P均<0.05);相关性分析显示 EDN 组和CDN组血清 FFA、Hcy、CysC 水平与 UAER 均呈高度正相关(P<0.01)。结论:FFA、Hcy、CysC 水平升高与DN的发展进程有关。

목적:관찰불동분기당뇨병신병(DN)환자혈청유리지방산(FFA)、동형반광안산(Hcy)、광억소 C(CysC)등수평변화급여뇨미량백단백배설솔(UAER)적상관성。방법:2형당뇨병(T2DM)환자178례,근거24h UAER분위단순T2DM조(DM조,n=82)、조기DN조(EDN,n=56);림상DN조(CDN,n=40례);동기체검건강자작위대조조(n=42)。채용전자동생화분석의측정각조혈청 FFA(매법)、Hcy(순배매법)、CysC(면역증강비탁법)、뇨소담(Urea,매법)、기항(Cr,매법)함량,채용전자동특충단백분석의측정 UAER,비교각조각지표수평차이,분석EDN、CDN조환자 FFA、Hcy、CysC여 UAER적상관성。결과:EDN 조화 CDN 조혈청 FFA、Hcy、CysC 수평고우DM조화대조조(P균<0.05),차CDN조고우 EDN조,DM조고우대조조(P균<0.05);상관성분석현시 EDN 조화CDN조혈청 FFA、Hcy、CysC 수평여 UAER 균정고도정상관(P<0.01)。결론:FFA、Hcy、CysC 수평승고여DN적발전진정유관。
Objective:To observe the changes of serum free fatty acid(FFA),homocysteine(Hcy),cystatin C(CysC)levels in diabetic nephropathies(DN)patients and the relationship with urinary albumin excretion rate(UAER).Method: 178 cases of type 2 diabetes mellitus(T2DM)were chosed,and they were divided into three groups,namely DM group (n =82),early dibetic nephropathy group(EDN,n =56)and clinical diabetes nephropathy group(CDN,n =40),according to UAER in 24 hours.42 healthy individuals were selected as the control group.The levels of serum FFA,Hcy,CysC,urea nitrogen(Urea)and creatinine(Cr)were detected by automatic biochemical analyzers, UAER were detected by automatic special protein analyzer.The relationship of FFA,Hcy,CysC and UAER in EDN,CDN patients were analyzed.Results:Serum FFA,Hcy and CysC levels were higher in EDN,CDN groups than DM and control groups (P <0.05),exhibiting CDN group > EDN group > DM group > control group(P < 0.05).Correlation analysis showed that blood FFA,Hcy,CysC and UAER had high degree of positive correlation between EDN group and CDN group(P <0.01).Conclusion:The development of DN was closely related to the increase of FFA,Hcy and CysC levels.
