目的:探讨颈根部涉及甲状腺各种占位性病变CT影像学特征,提高诊断准确性。方法回顾分析近5年皆有病理证实明确诊断的不同颈根部涉及甲状腺占位性病变的临床资料,重点分析14例不同颈根部涉及甲状腺占位性病变的CT影像学资料,比较不同占位性病变的CT影像学的特点。结果本组14例中3例为甲状腺乳头状癌侵犯颈段气管和/或颈段食管、4例为甲状腺B细胞型淋巴瘤、3例为颈段食管癌侵犯甲状腺、4例为巨大结节性甲状腺肿侵犯上纵隔。不同占位性病变有不同影像学特点。结论颈根部涉及甲状腺各种占位性病变的CT影像学有一定的特点,可以作为鉴别诊断的依据,深入了解各种占位性病变的CT影像学特点,可减少误诊的几率。

목적:탐토경근부섭급갑상선각충점위성병변CT영상학특정,제고진단준학성。방법회고분석근5년개유병리증실명학진단적불동경근부섭급갑상선점위성병변적림상자료,중점분석14례불동경근부섭급갑상선점위성병변적CT영상학자료,비교불동점위성병변적CT영상학적특점。결과본조14례중3례위갑상선유두상암침범경단기관화/혹경단식관、4례위갑상선B세포형림파류、3례위경단식관암침범갑상선、4례위거대결절성갑상선종침범상종격。불동점위성병변유불동영상학특점。결론경근부섭급갑상선각충점위성병변적CT영상학유일정적특점,가이작위감별진단적의거,심입료해각충점위성병변적CT영상학특점,가감소오진적궤솔。
Objective To investigate the CT imaging features of cervical roots involving various thyroid lesions for improving the diagnostic accuracy. Methods We analyzed clinical datas of cervical roots involving various thyroid lesions in recent five years retrospectively, which were confirmed diagnosis by pathology. 14 cases of cervical root placeholder thyroid diseases were analyzed and the different characteristics of space-occupying lesions of the CT imaging were compared carefully. Results Of 14 cases, there are 3 cases of thyroid papillary carcinoma which invaded cervical trachea and/or neck segment esophageal, 4 cases of type B cell lymphoma, 3 cases of cervical esophageal cancer which invaded thyroids and 4 cases of large nodular goiter which invaded superior mediastinum. The CT imaging characteristics were different. Conclusions CT imagings of cervical roots lesions involved all kinds of space-occupying lesions of the thyroid gland have certain characteristics, which can be used as the basis for differential diagnosis. In-depth understanding of the CT imaging characteristics of various space-occupying lesions can reduce the misdiagnosis rate.
