目的:探讨声触诊组织成像技术定性诊断在乳腺实性小结节中的应用价值。方法对57例患者共68个乳腺实性小结节进行常规超声及VTI检查,经VTI图像对乳腺实性小结节进行评分。所用病例均行组织病理学检查,以此为据绘制ROC曲线,并计算其敏感度、特异性及准确率。结果68个乳腺小结节经组织病理学诊断后,证实41个为良性,27个为恶性。乳腺实性小结节VTI评分4分及以上者,恶性病例发生率85.2%明显高于低评分组(P<0.05)。VTI图像与二维图像中结节的面积比,乳腺恶性结节明显高于良性结节(P<0.05)。以面积比1.47为节点诊断乳腺恶性实性小结节敏感度、特异性分别为88.5%、91.8%。结论VTI技术无创,在乳腺实性小结节鉴别诊断中有较高价值。

목적:탐토성촉진조직성상기술정성진단재유선실성소결절중적응용개치。방법대57례환자공68개유선실성소결절진행상규초성급VTI검사,경VTI도상대유선실성소결절진행평분。소용병례균행조직병이학검사,이차위거회제ROC곡선,병계산기민감도、특이성급준학솔。결과68개유선소결절경조직병이학진단후,증실41개위량성,27개위악성。유선실성소결절VTI평분4분급이상자,악성병례발생솔85.2%명현고우저평분조(P<0.05)。VTI도상여이유도상중결절적면적비,유선악성결절명현고우량성결절(P<0.05)。이면적비1.47위절점진단유선악성실성소결절민감도、특이성분별위88.5%、91.8%。결론VTI기술무창,재유선실성소결절감별진단중유교고개치。
Objective To evaluate the diagnostic value of virtual touch tissue imaging (VTI) on breast nodules .Methods A total of 68 beast nodules in 57 patients were examined with ultrasonography and VTI .Differences of VTI grade between benign and malignant lesions of VTI were analyzed by receiver operator characteristic (ROC) curve ,and the sensitivity ,specificity and accuracy of VTI were calculated .Results There were 41 benign lesions and 27 malignant ones among all nodules .85.2% of all nodules marked more than 4 were proved malignant ,which were higher than nodules marked lower 4 (P<0.05) .The area ratio of malignant nodules was higher than that of benign ones(P<0.05) .1.47 as the Cut-off point of area ratio ,sensitivity and spe-cificity of diagnosing breast cancer were 88.5% ,91.8% .Conclusions VTI can be used as a noninvasive and effective means for the differential diagnosis of breast nodules .
