目的:测定对多囊卵巢综合征(PCOS)患者血清中内脂素(visfatin)、游离脂肪酸(FFA)水平,研究两者之间的相关性及其与PCOS发病之间可能存在的关系。方法选择确诊PCOS的患者42例,按照体重指数分为肥胖组(1组)22例,非肥胖组(2组)20例;另因为女方输卵管因素或男方因素等来我院拟行试管的病人40例,同样按照体重指数进行划分,其中肥胖组(3组)20例,非肥胖组(4组)20例。抽取患者肘正中静脉血,对其血清中的visfatin、FFA、糖脂代谢指标等进行检测。结果visfatin在PCOS组高于对照组,1组高于2组,1组显著高于3、4组。FFA 实验组显著高于对照组。经spearman相关性分析显示,visfatin与 HDL负相关,与BM I、腰臀比、W、FINS、甘油三脂呈正相关,与其他指标无明显相关。游离脂肪酸与 HDL负相关,与TG、BMI、W、FINS、HOMI-IR、T、LPa呈正相关;Visfatin与FFA呈正相关,与其他指标无明显相关。结论 PCOS患者体内存在脂质代谢紊乱与糖代谢紊乱,PCOS肥胖型患者体内的糖脂代谢异常尤为明显,胰岛素抵抗程度更明显,内分泌激素的异常也更严重;visfatin与FFA呈正相关,均与HDL呈负相关,与TG、BMI、W、FINS、HOMI-IR呈正相关,且在PCOS患者血清中较对照组明显升高,提示两者在PCOS发病中起到一定的作用。

목적:측정대다낭란소종합정(PCOS)환자혈청중내지소(visfatin)、유리지방산(FFA)수평,연구량자지간적상관성급기여PCOS발병지간가능존재적관계。방법선택학진PCOS적환자42례,안조체중지수분위비반조(1조)22례,비비반조(2조)20례;령인위녀방수란관인소혹남방인소등래아원의행시관적병인40례,동양안조체중지수진행화분,기중비반조(3조)20례,비비반조(4조)20례。추취환자주정중정맥혈,대기혈청중적visfatin、FFA、당지대사지표등진행검측。결과visfatin재PCOS조고우대조조,1조고우2조,1조현저고우3、4조。FFA 실험조현저고우대조조。경spearman상관성분석현시,visfatin여 HDL부상관,여BM I、요둔비、W、FINS、감유삼지정정상관,여기타지표무명현상관。유리지방산여 HDL부상관,여TG、BMI、W、FINS、HOMI-IR、T、LPa정정상관;Visfatin여FFA정정상관,여기타지표무명현상관。결론 PCOS환자체내존재지질대사문란여당대사문란,PCOS비반형환자체내적당지대사이상우위명현,이도소저항정도경명현,내분비격소적이상야경엄중;visfatin여FFA정정상관,균여HDL정부상관,여TG、BMI、W、FINS、HOMI-IR정정상관,차재PCOS환자혈청중교대조조명현승고,제시량자재PCOS발병중기도일정적작용。
Objective To determinate visfatin and free fatty acids in the serum of polycystic ovary syndromed (PCOS) pa-tients ,researching the relationship between them and the possible relationship with pathogenesis of PCOS .Methods According to the body mass index ,42 cases of patients with PCOS were selected ,which was divided into obese group (group 1) 22 cases and non obesity group (group 2) 20 cases .The other 40 patients as an control group ,who was intending to undergoing IVF ,was divided into the obese group (Group 3) 20 cases ,non obesity group (Group 4) 20 ,because female tubal factor (the fallopian tube water or blocking ) or male factors (oligospermia ,asthenospermia) ,equally according to body mass index .patients venous blood of Zhou Zheng zhong were selected ,the visfatin ,free fatty acids ,glucose and lipid metabolism in serum was detected .Re-sults Visfatin in PCOS group was higher than that in control group ,the visfatin of 1 group was higher than that in group 2 ,the visfatin of 1 group was significantly higher than that of 3 ,4 group .The FFA group was significantly higher than the control group .The Spearman correlation analysis showed that ,visfatin was negatively correlated with HDL ,and was positively correla-ted with BMI ,waist hip ratio ,W ,FINS ,glycerin three greases .and other indexes had no significant correlation .Free fatty acids was negatively correlated with HDL ,and was positively correlated with TG ,BMI ,W ,FINS ,HOMI-IR ,T ,LPa .Visfatin was positively correlated with FFA ,and with other indexes had no significant correlation .Conclusion Lipid and glucose metabolism disorder in PCOS patients in vivo is present ,glucose and lipid metabolism in obese PCOS patients is obvious ,the degree of in-sulin resistance is more obvious ,endocrine abnormalities were more severe ;visfatin was positively associated with FFA ,was negatively correlated with HDL ,was positively correlated with TG ,BMI ,and W ,FINS ,HOMI-IR .Visfatin and FFA in serum of the PCOS patients was significantly higher than the control group ,which suggested that they play a role in the pathogenesis of PCOS .
