目的:评价双时相显像在胰腺良恶性鉴别诊断中的应用价值。方法对24例胰腺癌患者和10例胰腺良性病变患者进行18F-FDG PET双时相显像(早注射FDG后50~60 min行早期显像,注射后120~150 min行延迟显像),测定两次显像病灶的标准摄取值(standard uptake value,SUV),并计算SUV变化率(ΔSUV)。结果胰腺癌组SUV 早期显像=4.63±2.09,SUV 延迟显像=5.86±2.74,ΔSUV=(24.76±15.58)%,差异有显著性(t=7.94,P<0.01)。而胰腺良性病变SUV 早期显像=2.48±0.95,SUV 延迟显像=2.81±1.18,ΔSUV=(12.66±20.97)%,差异有显著性(t=2.67,P<0.05),且胰腺癌组SUV 延迟显像明显高于良性病变组(t=2.25,P<0.05)。结论胰腺恶性病变SUV 延迟显像升高的幅度高于良性病变,但黏液腺癌SUV 延迟显像升高的幅度低于良性病变,总体而言,双时相显像方法简便,在临床上具有一定的实用价值。

Objective To assess the clinical value of dual-time point 18F-FDG PET imaging for differential diag-nosis between benign pancreatic mass and pancreaticcancer. Method Twenty-four patients with pancreatic cancer and 10 patients with benign pancreatic mass underwent dual-time 18F-FDG PET imaging. Prophase scans and delayed scans were administered 50-60 min and 120-150 min after the intravenous injection of 18F-FDG, respectively. The maximum standardized uptake value (SUV) and SUVvariation (ΔSUV) were calculated for the twotime points. Re-sult In pancreatic cancer group, SUVprophase= 4.63±2.09, SUVdelayed= 5.86±2.74, and ΔSUV =(24.76±15.58)%, with sig-nificant difference (t =7.94, P <0.01). While in benign pancreatic mass group,SUVprophase= 2.48±0.95, SUVdelayed= 2.81± 1.18, and ΔSUV =(12.66±20.97)%, with significant difference (t =2.67, P <0.05), and SUVdelayed in pancreatic cancer group was significantly higher than thati in benigh pancreatic mass group (t =2.25, P <0.05). Conclusion Compared with benign lesion, the elevatory extent of SUVdelayed for pancreatic malignant lesions ishigher, while it is lower in mu-cinous adenocarcinoma. As a convenient method, dual-time point18F-FDG PET imaging is recommended in clinical practice.
