目的：观察高强度聚焦超声( high intensity focused ultrasound,HIFU)联合米非司酮治疗子宫腺肌病的疗效。方法100例子宫腺肌病患者随机分为研究组( HIFU联合米非司酮治疗)和对照组(仅用HIFU治疗),比较两组患者治疗前后的临床症状、子宫体积及病灶大小等变化。结果治疗12月后所有患者中Ⅴ~Ⅵ级疼痛比例由治疗前的22%降至7%,子宫平均体积缩小18%,病灶体积缩小26%。两组比较,研究组患者在治疗6月后及12月的痛经缓解率优于对照组( P＜0.05),研究组患者贫血缓解率高于对照组(P＜0.05)。结论 HIFU联合米非司酮及仅用HIFU治疗子宫腺肌病均有效,HIFU联合米非司酮治疗子宫腺肌病的近期疗效优于单纯HIFU治疗。
목적：관찰고강도취초초성( high intensity focused ultrasound,HIFU)연합미비사동치료자궁선기병적료효。방법100례자궁선기병환자수궤분위연구조( HIFU연합미비사동치료)화대조조(부용HIFU치료),비교량조환자치료전후적림상증상、자궁체적급병조대소등변화。결과치료12월후소유환자중Ⅴ~Ⅵ급동통비례유치료전적22%강지7%,자궁평균체적축소18%,병조체적축소26%。량조비교,연구조환자재치료6월후급12월적통경완해솔우우대조조( P＜0.05),연구조환자빈혈완해솔고우대조조(P＜0.05)。결론 HIFU연합미비사동급부용HIFU치료자궁선기병균유효,HIFU연합미비사동치료자궁선기병적근기료효우우단순HIFU치료。
Objective To evaluate the therapeutic effect of high intensity focused ultrasound ( HIFU ) with Mifepristone on adenomysis. Methods 100 patients with adenomyosis were randomly divided in study group using HIFU combine with Mifepristone and control group using HIFU only. The clinical symptom, uterine size and lesion size were compared between two groups. Results The percentage of patients withⅤ - Ⅵ dysmenorrhea was decreased from 22% to 7% after 12 months treatment, uterine size was reduced by 18% and the lesion size was reduced by 26%. The relief rate of dysmenorrhea in study group was significantly higher than control group after 6 to 12 months treatment(P<0. 05), the relief rate of anemia in study group was significantly higher than control group (P<0. 05). Conclusion Both HIFU and HIFU combined with Mifepristone are effective in treating adenomyosis. The therapeutic effect of HIFU combined with Mifepristone seems superior to using HIFU only.
