目的：探讨采用不同手术方式治疗育龄期妇女子宫肌瘤的临床疗效。方法将80例育龄期子宫肌瘤患者根据肌瘤的大小、位置及患者的意愿分为A、B、C 3组,A组采用经腹子宫肌瘤剔除(26例), B组经阴道子宫肌瘤剔除(27例),C组采用腹腔镜下子宫肌瘤剔除(27例),比较3组的临床疗效。结果 C组手术时间、出血量、肛门排气时间、住院时间少于A、B两组,住院费用高于A、B组(P＜0.05)；3组术后不良反应率分别为34.62%、51.85%、11.11%,C组低于A、B两组( P＜0.05)。结论腹腔镜下剔除育龄期子宫肌瘤具有疗效好、创伤小、不良反应少的特点,对于具有腹腔镜手术适应证的子宫肌瘤患者采用腹腔镜手术剔除肌瘤具有广泛的应用前景。
목적：탐토채용불동수술방식치료육령기부녀자궁기류적림상료효。방법장80례육령기자궁기류환자근거기류적대소、위치급환자적의원분위A、B、C 3조,A조채용경복자궁기류척제(26례), B조경음도자궁기류척제(27례),C조채용복강경하자궁기류척제(27례),비교3조적림상료효。결과 C조수술시간、출혈량、항문배기시간、주원시간소우A、B량조,주원비용고우A、B조(P＜0.05)；3조술후불량반응솔분별위34.62%、51.85%、11.11%,C조저우A、B량조( P＜0.05)。결론복강경하척제육령기자궁기류구유료효호、창상소、불량반응소적특점,대우구유복강경수술괄응증적자궁기류환자채용복강경수술척제기류구유엄범적응용전경。
Objective To explore the choice of operations on hysteromyomectomy of women of child-bearing age. Methods 80 cases of child-bearing age with hysteromyoma were divided into 3 groups according to the site and size of the tumor, group A (26 cases) for abdominal myomectomy, group B (27 cases) for uterine fibroids vaginal excision, group C(27 cases) for laparoscopic myomectomy, clinical efficacy of three groups were compared. Results The operation time, blood loss, exhaust time, length of hospital stay of group C was less than group A and group B, the costs of group C was higher than group A and group B(P<0. 05). Postoperative adverse reaction rate of 3 groups were 34. 62%, 51. 85%, 11. 11% respectively, the adverse reaction rate of group C was lower than the other 2 groups(P<0. 05). Conclusion Laparoscopic myomectomy of child-bearing age has a good effect with the characteristics of small trauma and less adoerse effects, which is suitable for patients with laparoscopic myomectomy indications.
