目的：比较使用 Hem-o-lok 结扎夹与传统结扎技术在Ⅱ、Ⅲ型食管胃结合部腺癌手术中的效果，分析结扎夹的优越性。方法随机将20例Ⅱ、Ⅲ型食管胃结合部腺癌需行近端胃切除术的患者分成两组，第一组术中使用 Hem-o-lok 结扎夹闭胃周血管，第二组使用传统结扎技术，比较两组间离断胃周血管时间及术中出血量。结果两组手术均成功，第一组离断胃周血管时间及术中出血量分别为（15.20±1.22）min 和（0.50±0.85）ml，均小于第二组的（24.60±1.50）min 和（2.20±1.48）ml（P ﹤0.05）。结论Ⅱ、Ⅲ型食管胃结合部腺癌行近端胃切除术时，使用 Hem-o-lok 结扎夹在离断胃周血管时间及术中出血量方面优于传统结扎技术。
목적：비교사용 Hem-o-lok 결찰협여전통결찰기술재Ⅱ、Ⅲ형식관위결합부선암수술중적효과，분석결찰협적우월성。방법수궤장20례Ⅱ、Ⅲ형식관위결합부선암수행근단위절제술적환자분성량조，제일조술중사용 Hem-o-lok 결찰협폐위주혈관，제이조사용전통결찰기술，비교량조간리단위주혈관시간급술중출혈량。결과량조수술균성공，제일조리단위주혈관시간급술중출혈량분별위（15.20±1.22）min 화（0.50±0.85）ml，균소우제이조적（24.60±1.50）min 화（2.20±1.48）ml（P ﹤0.05）。결론Ⅱ、Ⅲ형식관위결합부선암행근단위절제술시，사용 Hem-o-lok 결찰협재리단위주혈관시간급술중출혈량방면우우전통결찰기술。
Objective To discuss the superiority of Hem-o-lock ligation chip in the surgical therapy of the patients with typeⅡand Ⅲ adenocarcinoma at esophagogastric junction. Methods 20 patients with type Ⅱ or type Ⅲ adenocarcinoma at esophagogastric junction to receive distal gastrectomy were randomly divided into 2 groups;Hem-o-lock ligation chip was used in the operation to close the gastric vessels,while the traditional ligation technique was applied to the second group;a comparative study was made to the time of gastric vascular disconnection and the bleeding volume in operation between the 2 groups. Results The operations in both groups were successful;the time of gastric vascular disconnection and the bleeding volume in operation in the first group were(15. 20 ±1. 22)min and (0. 50 ±0. 85)ml,lower than those in the second group:(24. 60 ± 1. 50)min and(2. 20 ± 1. 48)ml(P ﹤ 0. 05). Conclusions Hem-o-lock ligation chip is superior to the traditional ligation technique in the time of gastric vascular disconnection and the bleeding volume in operation in the distal gastrectomy of adenocarcinoma at esophagogastric junction typeⅡand type Ⅲ.
