目的:通过SHEL ( soft hard environment litigant )模式对药房自动化设备与传统药品管理模式相结合存在的问题进行分析并加以改进,减少全自动口服摆药机药品发放差错,加强药品管理,提高用药安全。方法回顾性分析医院2011年12月至2013年11月住院部药房全自动口服摆药机单剂量发放药品的差错68例,针对全自动单剂量口服摆药机药品发放差错的因素进行分类,应用SHEL模式综合评价药品发放差错的原因,提出相关改进措施。结果 SHEL模式对全自动单剂量口服摆药机药品发放差错的因素进行分析并改进后,住院药房药品发放差错数发生率降低了78.57%(χ2=8.526,P=0.036),提高了药品安全管理。结论运用SHEL模式能明确全自动单剂量口服摆药机安全管理中的缺陷,找出解决方案,提高药房自动化管理水平。

Objective To adopt the soft hard environment litigant ( SHEL ) pattern to analyze the problems existing in the combination of the pharmacy automatic equipment and the traditional drug management,and to make the improvement for reducing the dispensing errors of the automatic tablet dispensing machine, strengthening the drug management and increasing the medication safety. Methods 68 cases of single dose dispensing errors caused by the automatic tablet dispensing machine in the inpatients pharmacy of the hospital from De-cember 2011 to November 2013 were retrospectively analyzed and classified according to the factors aiming at the errors caused by the automatic tablet dispensing machine. The SHEL pattern was adopted to evaluate the causes of the drug dispensing errors and the related improvement measures were proposed. Results By analyzing the factors causing the dispensing errors of the automatic tablet dispensing machine by the SHEL pattern and after improvement, the error occurrence rate of the drug dispensing in the inpatients pharmacy was reduced by 78. 57% (χ2=8. 526,P=0. 036 ) . The drug safety management was increased. Conclusion Adopting the SHEL pattern can determine the defects of the automatic tablet dispensing machine in the drug safety management, find out the solution scheme and in-crease the pharmacy automatic management level.
