目的:探讨肝性脊髓病( HM )颅脑MRI表现及其临床意义。方法:确诊为肝性脊髓病患者11例,年龄31~62岁。有慢性肝硬变病史1~9年,乙肝后肝硬变6例,丙肝后肝硬变2例,慢性酒精性肝硬变3例,其中曾行脾切除及门腔分流术7例。所有患者均行CE1.5T超导MRI扫描、颅脑CT、MRI、肝功能和血氨等检查,4例行脊髓MRI。结果:11例患者中颅脑CT有轻度脑萎缩4例,脊髓MRI无异常信号7例。 MRI扫描结果,11例颅脑对称性双侧苍白球,6例于中脑大脑脚及内囊前肢T1 WI像不同程度的高信号,7例T2WI像和FLAIR无异常表现。结论:HM时颅脑 MRI苍白球特征性改变是T1WI高信号,脊髓MRI没有明显的变化。这一特征有助于HM的诊断。

목적:탐토간성척수병( HM )로뇌MRI표현급기림상의의。방법:학진위간성척수병환자11례,년령31~62세。유만성간경변병사1~9년,을간후간경변6례,병간후간경변2례,만성주정성간경변3례,기중증행비절제급문강분류술7례。소유환자균행CE1.5T초도MRI소묘、로뇌CT、MRI、간공능화혈안등검사,4례행척수MRI。결과:11례환자중로뇌CT유경도뇌위축4례,척수MRI무이상신호7례。 MRI소묘결과,11례로뇌대칭성쌍측창백구,6례우중뇌대뇌각급내낭전지T1 WI상불동정도적고신호,7례T2WI상화FLAIR무이상표현。결론:HM시로뇌 MRI창백구특정성개변시T1WI고신호,척수MRI몰유명현적변화。저일특정유조우HM적진단。
Objective:To explore the brain ’ s Magnetic Resonance Imaging features and clinical significance of patients with hepatic myelopathy .Methods:11 patients with Hepatic Myelopathy whose age were 31~62 years old were selected as objective of the study .They had 1 to 9 years ’ history of chronic cirrhosis . There were 2 cases of liver cirrhosis after hepatitis B , 2 cases of liver cirrhosis after hepatitis C ,3 cases of chronic alcoholic liver cirrhosis , in which 7 cases had undergone splenectomy and portosystemic shunt .All patients were carried out the examinations of CE1.5T superconducting MRI scan,brain CT,MRI,and other liver function, blood ammonia .4 cases of them were examed through spinal cord MRI .Results:In 11 patients, there were 4 cases with mild brain atrophy illustrated by brain CT , and 7 cases without abnormal signal demonstrated by spinal MRI .MRI scan showed that 11 cases had symmetrically bilateral globus pallidus feature , 6 cases had different levels ’ T1 WI high signal in cerebral pedunculus of midbrain and anterior limb , and 7 patients had no abnormalities on T 2 WI images and FLAIR .Conclusion:The characteristic changes of globus pallidus in hepatic myelopathy are T 1 WI high signal , and spinal image have not change significantly in MRI.This feature can help the diagnosis of Hepatic myelopathy .
