目的:探讨磁共振三维稳态进动快速成像(three-dimensional fast imaging employing steady state acquisition ,3D-FI-ESTA)对原发性三叉神经痛的诊断价值。方法采用磁共振3D-FIESTA序列扫描52例原发性三叉神经痛患者的三叉神经出脑干段,并做多平面重建。分析患侧组及健侧组三叉神经出脑干段与邻近血管的关系。结果患侧组与健侧组神经血管接触阳性比例分别为52/53与35/51,神经受压推移比例分别为10/53与1/51,差异有统计学意义( P<0.05)。结论磁共振3D-FIESTA,可以清晰显示三叉神经与邻近血管的关系,为原发性三叉神经痛的诊断及治疗提供可靠的影像学依据。

목적:탐토자공진삼유은태진동쾌속성상(three-dimensional fast imaging employing steady state acquisition ,3D-FI-ESTA)대원발성삼차신경통적진단개치。방법채용자공진3D-FIESTA서렬소묘52례원발성삼차신경통환자적삼차신경출뇌간단,병주다평면중건。분석환측조급건측조삼차신경출뇌간단여린근혈관적관계。결과환측조여건측조신경혈관접촉양성비례분별위52/53여35/51,신경수압추이비례분별위10/53여1/51,차이유통계학의의( P<0.05)。결론자공진3D-FIESTA,가이청석현시삼차신경여린근혈관적관계,위원발성삼차신경통적진단급치료제공가고적영상학의거。
Objective To evaluate the diagnostic value of MR three-dimensional fast imaging employing steady state acquisi-tion (3D-FIESTA) in patients with idiopathic trigeminal neuralgia .Methods The 3D-FIESTA and multi-plane reconstruction (MPR) were performed in 52 patients with idiopathic trigeminal neuralgia .MR images were analyzed to determine the relationship between the brain stem segment of trigeminal nerves and micro vessels of bilateral sides .Results Neurovascular contact could be found in 52/53 cases on the symptomatic sides and 35/51cases on the asymptomatic sides .Nerve compression or displacement was identified in 10 pa-tients on the symptomatic sides ,and 1 patient on asymptomatic sides .The differences between the neurovascular contact and nerve com-pression were significant (P<0.05).Conclusion MR 3D-FIESTA can clearly demonstrate the relationship between the nerve and surrounding blood vessels and provide credible imaging evidence for pathogenic diagnosis and clinical treatment planning .
