目的:检测慢性肾脏病(CKD)患者肾组织胸腺肽β4(Tβ4)的表达,并探讨其临床意义。方法选择广州市第一人民医院2010-2012年收治的 CKD 患者84例,根据肾小管间质纤维化分级标准分为0级组19例、Ⅰ级组25例、Ⅱ级组20例、Ⅲ级组20例;选择同期因创伤而在我院行肾切除术患者8例为对照组。收集所有患者肾组织标本,采用 Envision 免疫组化二步法检测 Tβ4和α血管平滑肌肌动蛋白(α- SMA)的表达。结果对照组、0级组、Ⅰ级组、Ⅱ级组、Ⅲ级组 Tβ4 OD 值分别为(0.21±0.01)、(0.25±0.03)、(0.30±0.03)、(0.33±0.02)、(0.36±0.04),α- SMA OD 值分别为(0.22±0.02)、(0.24±0.01)、(0.29±0.04)、(0.35±0.02)、(0.39±0.04);对照组 Tβ4 OD 值﹤0级组﹤Ⅰ级组﹤Ⅱ级组﹤Ⅲ级组(P ﹤0.05),对照组与0级组α- SMA OD 值﹤Ⅰ级组﹤Ⅱ级组﹤Ⅲ级组(P ﹤0.05),而对照组α- SMA OD 值与0级组比较,差异无统计学意义(P ﹥0.05)。Pearson 相关分析结果显示,肾小管间质纤维化程度与 Tβ4表达水平、α- SMA 表达水平均呈正相关(r =0.742、0.907,P ﹤0.01);肾小管间质 Tβ4表达水平与α- SMA 表达水平亦呈正相关(r =0.767,P ﹤0.01)。结论 Tβ4在肾小管间质纤维化的发生和发展过程中发挥着重要作用,可能参与了上皮细胞间质转分化(EMT)。

Objective To detect the expression of thymosinβ4 in renal tissues in patients with chronic kidney disease (CKD),to explore its clinical significance. Methods From 2010 to 2012,a total of 84 CKD patients were selected,and then they were divided into 4 groups according to Renal Tubular Interstitial Fibrosis Grading Standard:0 - grade group(n = 19),Ⅰ- grade group(n = 25),Ⅱ - grade group(n = 20),Ⅲ - grade group(n = 20). At the same time,8 cases underwent ne-phrectomy caused by trauma were selected as control group. Envision immunohistochemical two - step method was used to detect the expression of thymosinβ4 and α - vascular smooth muscle actin( α - SMA). Results The OD of thymosinβ4 of control group,0 - grade group,Ⅰ - grade group,Ⅱ - grade group,Ⅲ - grade group was(0. 21 ± 0. 01),(0. 25 ± 0. 03),(0. 30 ± 0. 03),(0. 33 ± 0. 02),(0. 36 ± 0. 04),respectively,and control group ﹤ 0 - grade group ﹤ Ⅰ - grade group ﹤ Ⅱ -grade group ﹤ Ⅲ - grade group(P ﹤ 0. 05);the OD of α - SMA of control group,0 - grade group,Ⅰ - grade group,Ⅱ -grade group,Ⅲ - grade group was(0. 22 ± 0. 02),(0. 24 ± 0. 01),(0. 29 ± 0. 04),(0. 35 ± 0. 02),(0. 39 ± 0. 04),re-spectively,and control group or 0 - grade group 4 ﹤ Ⅰ - grade group 4 ﹤ Ⅱ - grade group 4 ﹤ Ⅲ - grade group(P ﹤ 0. 05), but there was no significant difference between control group and 0 - grade group( P ﹥ 0. 05). Pearson correlation analysis showed that,renal tubular interstitial fibrosis grading was positively correlated with the expression of thymosin β4 and α - SMA (r = 0. 742,0. 907,P ﹤ 0. 01);the expression of thymosin β4 was positively correlated with the expression of α - SMA(r =0. 767,P ﹤ 0. 01). Conclusion Thymosin β4 play an important role in the occurence and development of renal tubular intersti-tial fibrosis,may take participate in epithelial - mesenchymal transition(EMT).
