目的:探讨由益生菌、谷氨酰胺、深海鱼油、肠内营养混悬液(能全力)组成的免疫微生态肠内营养对合并糖尿病的胃肠肿瘤患者肠耐受性和脑肠肽Ghrelin的影响。方法92例合并糖尿病的胃肠肿瘤患者随机分为普通肠内营养组(对照组)和加入益生菌、谷氨酰胺、深海鱼油、能全力的免疫微生态肠内营养组(治疗组),各46例,两组均在手术中摆放鼻肠营养管,营养管放入空肠起始处,治疗组术后均给予免疫微生态营养,对照组给予能全力。观察两组对肠内营养的耐受性,术后第3天和第7天空腹抽血测定血浆活性Ghrelin,最后将两组结果进行对比分析。结果治疗组对肠内营养的耐受性优于对照组,并且术后第7天血浆活性Ghrelin高于对照组,差异有统计学意义(P<0.05)。结论对于合并糖尿病的胃肠肿瘤患者,应用由益生菌、谷氨酰胺、深海鱼油、能全力组成的免疫微生态肠内营养,能提高血浆活性Ghrelin,并能提高患者对肠内营养的耐受性。

목적:탐토유익생균、곡안선알、심해어유、장내영양혼현액(능전력)조성적면역미생태장내영양대합병당뇨병적위장종류환자장내수성화뇌장태Ghrelin적영향。방법92례합병당뇨병적위장종류환자수궤분위보통장내영양조(대조조)화가입익생균、곡안선알、심해어유、능전력적면역미생태장내영양조(치료조),각46례,량조균재수술중파방비장영양관,영양관방입공장기시처,치료조술후균급여면역미생태영양,대조조급여능전력。관찰량조대장내영양적내수성,술후제3천화제7천공복추혈측정혈장활성Ghrelin,최후장량조결과진행대비분석。결과치료조대장내영양적내수성우우대조조,병차술후제7천혈장활성Ghrelin고우대조조,차이유통계학의의(P<0.05)。결론대우합병당뇨병적위장종류환자,응용유익생균、곡안선알、심해어유、능전력조성적면역미생태장내영양,능제고혈장활성Ghrelin,병능제고환자대장내영양적내수성。
Objective To investigate the effect of an microbial immune enteral nutrition by probiotics and deep sea fish oil and glutamine and nutrison on the postoperative gastrointestinal cancer patients with diabetes in the tolerance and ghrelin.Methods 92 Gastrointestinal cancer patients with diabetes were randomized into the microbial immune enteral nutrition group and the control group, treatment of 46 patients, 46 patients in control group. All patients were placed in the surgery of nasal intestinal feeding tube, feeding tube placed in the jejunum beginning. The immune microbial nutrition was given to the study group, but not to the control group. Ghrelin concentration in blood of the two groups were detected on the 3, 7 days after operation, and the tolerance of the two groups for enteral nutrition were collected and then comparing the two indicators and get results.Results Microbial immune enteral nutrition group's ghrelin concentration in blood was higher than the control group on the 7 days after operation; And the tolerance of Microbial immune enteral nutrition group was better than the control group. Conclusion To the gastrointestinal cancer patients with diabetes, microbial immune enteral nutrition, could improve the tolerance of enteral nutrition and the ghrelin concentration in blood on the 7 days after operation.
