目的：探讨64排CT三维重建技术在股骨粗隆间骨折中的临床应用价值。方法：选取2012年7月-2013年12月笔者所在医院收治的经X线片证实为单侧粗隆间骨折的80例患者，对其进行X线片、64排CT扫描，记录X线片、64排CT扫描结果，对其股骨粗隆间骨折块数、骨折骨皮质连续性及骨折端稳定性评估进行统计。结果：股骨粗隆间骨折块数方面，64排CT扫描与术中结果比较差异无统计学意义(t=0.16， P>0.05)，与X线片比较差异有统计学意义(t=6.63，P<0.05)。骨折骨皮质连续性方面，64排CT与X线片比较差异有统计学意义(χ2=13.64，P<0.05)；骨折端稳定性评估结果方面，64排CT与X线片比较差异有统计学意义(χ2=26.97，P<0.05)。结论：64排CT三维重建能够更准确显示股骨粗隆间骨折的解剖学变化，更能确切的体现出骨折部位的真实情况及稳定性、骨折块的数量。对选择正确的治疗方法和内固定方式有重要的临床应用价值。
목적：탐토64배CT삼유중건기술재고골조륭간골절중적림상응용개치。방법：선취2012년7월-2013년12월필자소재의원수치적경X선편증실위단측조륭간골절적80례환자，대기진행X선편、64배CT소묘，기록X선편、64배CT소묘결과，대기고골조륭간골절괴수、골절골피질련속성급골절단은정성평고진행통계。결과：고골조륭간골절괴수방면，64배CT소묘여술중결과비교차이무통계학의의(t=0.16， P>0.05)，여X선편비교차이유통계학의의(t=6.63，P<0.05)。골절골피질련속성방면，64배CT여X선편비교차이유통계학의의(χ2=13.64，P<0.05)；골절단은정성평고결과방면，64배CT여X선편비교차이유통계학의의(χ2=26.97，P<0.05)。결론：64배CT삼유중건능구경준학현시고골조륭간골절적해부학변화，경능학절적체현출골절부위적진실정황급은정성、골절괴적수량。대선택정학적치료방법화내고정방식유중요적림상응용개치。
Objective:Investigate the clinical value of 64-slice CT three-dimensional reconstruction technique in intertrochanteric fracture classification. Method:From July 2012 to December 2013 in author’s hospital were treated by X-ray confirmed unilateral intertrochanteric fractures of 80 patients,X-ray, CT scan on the 64 row,64 row recording X-ray,CT scan,the cortical fracture stability evaluation andstatistics,the number of femur fracture intertrochanteric fracture.Result:The number of intertrochanteric fracture of femur,64 slice CT scan and intraoperative results had no significant difference(t=0.16,P>0.05), compared with X-ray statistical(t=6.63,P<0.05).Bonecortical continuity,there was statistical significance of 64 slice CT and X-ray difference(χ2=13.64, P<0.05);fracture stability assessment results,with statistical significance of 64 slice CT and X-ray difference(χ2=26.97,P<0.05).Conclusion:64-slice CT three-dimensional reconstruction technique can more accurately display anatomical changes intertrochanteric fracture,more exactly reflect the real situation and the stability of the fracture site,fracture of the number of blocks,has important clinical value to select the correct treatment methods and internal fixation.
