目的:探讨NTRAP阻石篮在输尿管镜钬激光碎石术中的价值和安全性。方法:采集2010年1月-2014年4月中50例输尿管上段结石患者,按随机数字表法为为阻石篮组19例和对照组31例。阻石篮组以输尿管镜在斑马导丝引导下进入输尿管见到结石后,于输尿管镜工作通道内放置NTRAP阻石篮,以钬激光完全粉碎结石至直径小于2 mm碎片后松开阻石篮,拔除阻石篮后留置双J管。术后第1天复查KUB,双J管于术后1~3个月拔除。对照组不使用NTRAP阻石篮,单纯使用钬激光碎石。比较两组的治疗效果。结果:NTRAP阻石篮组一次性碎石成功率达89.4%,高于对照组的74.2%,两组比较差异有统计学意义(P<0.05)。NTRAP阻石篮组的住院天数、手术时间均明显优于对照组(P<0.05)。NTRAP阻石篮组无结石移位,对照组6例出现结石移位。两组术后均无严重并发症。结论:NTRAP阻石篮操作简单、方便,一次性碎石成功率较高,可对输尿管结石产生有效封堵,预防输尿管结石碎石术中结石移位。

목적:탐토NTRAP조석람재수뇨관경화격광쇄석술중적개치화안전성。방법:채집2010년1월-2014년4월중50례수뇨관상단결석환자,안수궤수자표법위위조석람조19례화대조조31례。조석람조이수뇨관경재반마도사인도하진입수뇨관견도결석후,우수뇨관경공작통도내방치NTRAP조석람,이화격광완전분쇄결석지직경소우2 mm쇄편후송개조석람,발제조석람후류치쌍J관。술후제1천복사KUB,쌍J관우술후1~3개월발제。대조조불사용NTRAP조석람,단순사용화격광쇄석。비교량조적치료효과。결과:NTRAP조석람조일차성쇄석성공솔체89.4%,고우대조조적74.2%,량조비교차이유통계학의의(P<0.05)。NTRAP조석람조적주원천수、수술시간균명현우우대조조(P<0.05)。NTRAP조석람조무결석이위,대조조6례출현결석이위。량조술후균무엄중병발증。결론:NTRAP조석람조작간단、방편,일차성쇄석성공솔교고,가대수뇨관결석산생유효봉도,예방수뇨관결석쇄석술중결석이위。
Objective:To evaluate the efficacy and safety of NTRAP Retrieval Basket in ureteroscopic Holium laser lithotripsy for upper ureteral stone.Method:From January 2010 to April 2014,50 cases of upper ureteral stone were selected,and they were randomly divided into the NTRAP group(n=19)and the control group(n=31). The NTRAP group was treated with ureteroscope insert through the ureter to find stones aided by zebra guide wire,and then NTRAP retrieval basket was placed in the working channel of ureteroscope.When stones were crushed into fragments of 1-2 mm in diameter by lumenis holmium laser,the device was removed. The Double-J tube was placed.X ray was conducted and Double-J tube was retained in all the patients for 1-3 months postoperatively.The lumenis holmium laser without NTRAP retrieval basket was used in the control group.Result:One-time gravel success rate of the NTRAP group was 89.4%, and was higher than the 74.2%in the control group,there was significant difference(P<0.05). The hospitalization days and operation time in the NTRAP group were better than the control group(P<0.05). The NTRAP group was no stone shift,the control group had 6 cases. There were no serious postoperative complications of two groups .Conclusion:The application of NTRAP retrieval basket in ureteroscopic holium laser lithotripsy for upper ureteral stone is a highly effective and safe therapy and one-time gravel success rate is more higher. It can prevent stones from drifting during lithotripsy.
