目的：探讨术前血必净注射液治疗对结核毁损肺单肺通气时炎性因子水平的影响。方法：收治择期行单侧全肺切除术的肺结核病患者40例，随机分为对照组(A组)及治疗组(B组)，每组20例。A组给予常规胸外科术前准备，B组在A组基础上，术前1周开始给予中药血必净注射液50 mL静滴，2次/d。分别在入院时(T0)、麻醉诱导前(T1)、单肺通气60 min(T2)和120 min(T3)各时点抽取静脉血检测血浆TNF-α、IL-6、IL-8及IL-10水平。结果：与T0比较，两组 T2、T3时 TNF-α、IL-6及 IL-8水平均明显升高，差异有统计学意义(P＜0.05)；与 A 组比较，B 组 T2及 T3时TNF-α、IL-6及IL-8显著降低，差异有统计学意义(P＜0.05)。两组各时点IL-10无显著变化(P＞0.05)。结论：术前血必净治疗可抑制结核毁损肺患者单肺通气期间TNF-α、IL-6、IL-8水平的上升，减轻肺损伤早期的全身炎性反应。
목적：탐토술전혈필정주사액치료대결핵훼손폐단폐통기시염성인자수평적영향。방법：수치택기행단측전폐절제술적폐결핵병환자40례，수궤분위대조조(A조)급치료조(B조)，매조20례。A조급여상규흉외과술전준비，B조재A조기출상，술전1주개시급여중약혈필정주사액50 mL정적，2차/d。분별재입원시(T0)、마취유도전(T1)、단폐통기60 min(T2)화120 min(T3)각시점추취정맥혈검측혈장TNF-α、IL-6、IL-8급IL-10수평。결과：여T0비교，량조 T2、T3시 TNF-α、IL-6급 IL-8수평균명현승고，차이유통계학의의(P＜0.05)；여 A 조비교，B 조 T2급 T3시TNF-α、IL-6급IL-8현저강저，차이유통계학의의(P＜0.05)。량조각시점IL-10무현저변화(P＞0.05)。결론：술전혈필정치료가억제결핵훼손폐환자단폐통기기간TNF-α、IL-6、IL-8수평적상승，감경폐손상조기적전신염성반응。
Objective:To investigate the effect of preoperative Xuebijing injection in the treatment of inflammatory factor levels of tuberculosis destroyed lung during one lung ventilation.Methods: 40 cases with pulmonary tuberculosis who were scheduled for unilateral pneumonectomy.They were randomly divided into the control group(group A) and the treatment group(B group) with 20 cases in each.A group received routine preoperative preparation for department of thoracic surgery,while the B group were given Xue Bi Jing injection 50 mL intravenous drip,2 times/day before 1 weeks of the operation on the basis of A group.We venoued blood at admission time(T0),before induction of anesthesia(T1),single lung ventilation for 60 minutes(T2) and 120 minutes(T3) for each time point,and detected the levels of plasma TNF-α,IL-6,IL-8 and IL-10.Results:Compared with T0,the TNF-α, IL-6 and IL-8 at T2 and T3 were significantly increased,the difference was statistically significant(P<0.05);compared with the A group,the TNF-α,IL-6 and IL-8 of B group at T2 and T3 decreased significantly,and the difference was statistically significant(P<0.05).The IL-10 of two groups had no significant change(P>0.05).Conclusion:The preoperative treatment with Xue Bi Jing injection could inhibit the increase of TNF-α,IL-6,IL-8 levels of tuberculosis destroyed lung patients during one lung ventilation,and reduce the systemic inflammatory reaction of lung injury.
