目的探讨小凹蛋白1(Cav 1)在冠心病(CAD )合并2型糖尿病(T2DM )患者中的表达及其意义。方法选择148例经冠状动脉造影及临床证实的CAD患者,其中合并T2DM患者94例,单纯CAD54例,另选取无冠状动脉病变及糖尿病16例为对照组。用流式细胞技术检测各组外周血单核细胞 Cav 1的表达水平,全自动生化分析仪测定高敏C反应蛋白(hs CRP)及空腹血糖(FBG)。结果CAD合并T2DM患者外周血单核细胞Cav1表达水平显著低于对照组及单纯CAD组,差异有统计学意义(P<0.01,<0.05);CAD合并T2DM组血清hs CRP的水平显著高于单纯CAD组及对照组,差异有统计学意义(P<0.01)。单核细胞Cav1的表达水平与hs CRP及空腹血糖呈负相关(r=-0.47、-0.31,均P<0.01)。多元Logistic回归分析显示CAD合并T2DM与Cav 1及FBG密切相关(OR=0.97、3.44,均 P<0.05)。结论 Cav 1表达低下是发生CAD合并T2DM的一个危险因素,并在一定程度上可预测CAD合并T2DM的发生。

Objective To investigate the expression of Caveolin 1(Cav 1)in coronary artery disease (CAD) complicated with type 2 diabetes mellitus (T2DM ) .Methods Cav 1 level was measured in peripheral blood mononuclear cells using flow cytometry among 94 CAD complicated with T2DM patients and 54 CAD patients ,which CAD was diagnosed by coronary angiography and clinical manifes-tation,while16patientswithoutCADandT2DMwereenrolledasnormalcontrolgroup.High sensitivityCreactiveprotein(hs CRP) and fasting blood glucose (FBG) level were detected by automatic biochemistry analyzer .Results Compared with normal control and CAD group ,the expression of Cav 1 in CAD complicated with T2DM patients was significantly lower ( P<0 .01 ,<0 .05) and the lev-elofhs CRPinCADcomplicatedwithT2DMgroupwasextremelyhigher(P<0.01).Furthermore,theexpressionofCav1was negativecorrelatedwithhs CRPandFBG(r=-0.47、-0.31,P<0.01).ThemultiplelogisticregressionindicatedthatCAD complicated with T2DM and Cav 1、FBG were closely related(OR=0 .97、3 .44 ,P<0 .05) .Conclusions The lower level of expres-sion of Cav 1 on peripheral blood mononuclear cells is one risk factor for CAD complicated with T 2DM ,to some extent ,may be a possible predicting index for CAD with T2DM .
