目的:观察不同临床分期的2型糖尿病肾病( diabetic nephropathy, DN )尿液色素上皮衍生因子( PEDF)水平及其与病情严重程度的关系。方法选择2型糖尿病120例,根据病情分为单纯糖尿病组、DN Ⅲ期组、DNⅣ期组和DNⅤ期组4组,每组30例;另选择同期健康体检者30例作为对照组,观察比较5组尿液PDEF水平。结果尿液PDEF水平对照组为(1.13±0.39)μg/ml,单纯糖尿病组为(4.57±0.47)μg/ml,DNⅢ期组为(6.88±0.79)μg/ml,DNⅣ期组为(8.52±0.77)μg/ml,DNⅤ期组为(5.70±0.64)μg/ml。与对照组比较,单纯糖尿病组、DNⅢ期组、DNⅣ期组和DNⅤ期组尿液PDEF水平均升高,差异均有统计学意义(P<0.05);单纯糖尿病组、DNⅢ期组和DNⅣ期组随病情严重程度增加尿液PDEF水平升高,3组间两两比较差异亦均有统计学意义(P<0.05);DNⅤ期组尿液PEDF水平显著低于DNⅢ期组和DN Ⅳ期组,但高于单纯糖尿病组,差异亦均有统计学意义(P<0.05)。结论 DN患者尿液PEDF水平变化出现于病变早期,并且随着病情进展逐渐改变。尿液PEDF可作为早期诊断DN的一个敏感性指标。

목적:관찰불동림상분기적2형당뇨병신병( diabetic nephropathy, DN )뇨액색소상피연생인자( PEDF)수평급기여병정엄중정도적관계。방법선택2형당뇨병120례,근거병정분위단순당뇨병조、DN Ⅲ기조、DNⅣ기조화DNⅤ기조4조,매조30례;령선택동기건강체검자30례작위대조조,관찰비교5조뇨액PDEF수평。결과뇨액PDEF수평대조조위(1.13±0.39)μg/ml,단순당뇨병조위(4.57±0.47)μg/ml,DNⅢ기조위(6.88±0.79)μg/ml,DNⅣ기조위(8.52±0.77)μg/ml,DNⅤ기조위(5.70±0.64)μg/ml。여대조조비교,단순당뇨병조、DNⅢ기조、DNⅣ기조화DNⅤ기조뇨액PDEF수평균승고,차이균유통계학의의(P<0.05);단순당뇨병조、DNⅢ기조화DNⅣ기조수병정엄중정도증가뇨액PDEF수평승고,3조간량량비교차이역균유통계학의의(P<0.05);DNⅤ기조뇨액PEDF수평현저저우DNⅢ기조화DN Ⅳ기조,단고우단순당뇨병조,차이역균유통계학의의(P<0.05)。결론 DN환자뇨액PEDF수평변화출현우병변조기,병차수착병정진전축점개변。뇨액PEDF가작위조기진단DN적일개민감성지표。
Objective To observe the expression level of urinary pigment epithelium-derived factor ( PEDF) at differ-ent stages of type 2 diabetic nephropathy ( DN) and its relationship to disease condition. Methods 120 type 2 diabetic pa-tients were divided into four groups(30 patients per group), including simple diabetes mellitus group (T2DM group), type 2 diabetic nephropathy stage Ⅲ group( DN Ⅲ group) , type 2 diabetic nephropathy stage Ⅳ group( DN Ⅳ group) , and type 2 diabetic nephropathy stageⅤgroup( DNⅤgroup) . 30 healthy people with medical examinations at the same time were chosen as control group. Urinary PEDF in the 5 groups were detected for comparisons. Results The urinary PEDF was (1. 13 ±0.39)μg/ml in control group, (4. 57 ±0. 47)μg/ml in T2DM group, (6. 88 ±0. 79)μg/ml in DNⅢ group, (8. 52 ±0. 77)μg/ml in DNⅣ group and (5. 70 ±0. 64)μg/ml in DNⅤgroup, respectively. PEDF level in T2DM group, DNⅢgroup, DNⅣgroup and DNⅤgroup was significantly higher than that in the control group(P<0.05),and it increased with the severity of the disease in T2DM group, DNⅢgroup and DNⅣgroup, and the differences among the 3 groups were significant (P<0.05). PEDF level in DNⅤ group was lower than that in DNⅢ group and DNⅣ group, but higher than that in T2DM group, and the differences were significant ( P<0.05 ) . Conclusion The urinary PEDF level in patients with diabetic ne-phropathy may change in early phase and change gradually with progression of disease. It can be a sensitive marker in diagno-sing diabetic nephropathy at early stage.
