目的：探讨不同方法进行胆囊息肉样病变和胆囊良性肿瘤的鉴别诊断的差异。方法分析2011年1月-2013年6月间我院收治的胆囊息肉样病变患者156例，通过不同影像学检查获得筛查结果，将其与病理检查结果进行比对。结果与病理诊断结果比较，超声造影（χ2＝0．188，P ＞0．05），增强 CT（χ2＝2．308，P ＞0．05）及 MRI（χ2＝7．505，P ＞0．05）检出率均无显著差异，三组两两比较，超声造影检出率明显高于增强 CT（χ2＝4．769，P ＜0．05）及 MRI（χ2＝14．467，P ＜0．05）。结论超声造影能够细致地反映胆囊息肉样病变的周围血管情况及形态特征，其检出率最高，适合作为临床筛查诊断胆囊良性肿瘤的首选方法。
목적：탐토불동방법진행담낭식육양병변화담낭량성종류적감별진단적차이。방법분석2011년1월-2013년6월간아원수치적담낭식육양병변환자156례，통과불동영상학검사획득사사결과，장기여병리검사결과진행비대。결과여병리진단결과비교，초성조영（χ2＝0．188，P ＞0．05），증강 CT（χ2＝2．308，P ＞0．05）급 MRI（χ2＝7．505，P ＞0．05）검출솔균무현저차이，삼조량량비교，초성조영검출솔명현고우증강 CT（χ2＝4．769，P ＜0．05）급 MRI（χ2＝14．467，P ＜0．05）。결론초성조영능구세치지반영담낭식육양병변적주위혈관정황급형태특정，기검출솔최고，괄합작위림상사사진단담낭량성종류적수선방법。
Objective To explore the differential diagnosis between gallbladder polypoid le-sions and gallbladder benign tumor.Methods 156 patients with gallbladder polypoid lesions were examined through the different imaging method,and the results were compare with the pathological findings.Results Compared with the pathological diagnosis,the ultrasound imaging (χ2=0.188, P >0.188),enhanced CT (χ2= 2.308,P >0.05)and MRI (χ2=7.505,P >7.505)showed no significant differences in detection rate.Contrast-enhanced ultrasound was significantly better than enhanced CT (χ2=4.769,P <4.769)and MRI (χ2=14.467,P <0.05).Conclusion Contrast-enhanced ultrasound can carefully reflect the gallbladder polypoid lesions of peripheral vascular and morphological characteristics.Because its highest detection rate,it is suggested as the preferred method for the diagnosis of gallbladder benign tumor.
