目的:探讨无创正压通气治疗老年COPD合并呼吸衰竭患者的临床效果。方法选取我院收治的老年COPD并合并有呼吸衰竭的患者60例,随机分为A组与B组,B组患者行鼻导管持续低流量吸氧和常规药物治疗,A组患者在B组基础上增加机械通气面罩正压通气。比较两组患者血气分析结果、病死率、治疗周期和治疗后肺功能指标间的差异。结果 A组患者血气分析指标和治疗后肺功能指标显著优于B组( P<0.05),病死率显著低于B组(P<0.05),治疗周期显著短于B组(P<0.05)。结论无创正压通气可以有效提高老年COPD并发呼吸衰竭患者的临床治疗效果。

목적:탐토무창정압통기치료노년COPD합병호흡쇠갈환자적림상효과。방법선취아원수치적노년COPD병합병유호흡쇠갈적환자60례,수궤분위A조여B조,B조환자행비도관지속저류량흡양화상규약물치료,A조환자재B조기출상증가궤계통기면조정압통기。비교량조환자혈기분석결과、병사솔、치료주기화치료후폐공능지표간적차이。결과 A조환자혈기분석지표화치료후폐공능지표현저우우B조( P<0.05),병사솔현저저우B조(P<0.05),치료주기현저단우B조(P<0.05)。결론무창정압통기가이유효제고노년COPD병발호흡쇠갈환자적림상치료효과。
Objective To analysis the effect of noninvasive positive pressure mechanical ventilation on COPD elderly patients with respiratory failure. Methods 60 COPD elderly patients with respiratory failure were divided into objective group and control group.The control group was used conventional treatment plan and the objective group was additional noninvasive positive pressure mechanical ventilation.The difference of the oxygen content of blood,case fatality rate,treatment cycle time and the lung function indices between the two groups were compared. Results The oxygen content of blood and the lung function indices of the objective group were better than that in the control group(P < 0.05).The case fatality rate of the objective group was lower than that in the control group(P < 0.05). The treatment cycle time of the objective group was less than that in the control group(P<0.05). Conclusion It would get a satisfactory effect to use noninvasive positive pressure mechanical ventilation on COPD elderly patients with respiratory failure.
