目的:评价眼底血管荧光造影(FFA)在视网膜挫伤预后评价中的应用。方法选取视网膜挫伤患者51例为研究对象,分别采用视觉诱发电位(VEP)、FFA进行检查,并全部随访51例患者的视力变化。结果挫伤的眼睛其VEP P100波振幅明显降低,潜伏期明显延迟;治疗后VEP P100波振幅、潜伏期均明显改善(P<0.05);FFA检查混合性损伤32眼,单纯性损伤19眼,其中38眼表现为视网膜震荡,14眼表现为视网膜出血,5眼表现为脉络膜裂伤,4眼表现为黄斑裂孔,8眼表现为视神经挫伤。治疗后1个月复查眼底血管荧光造影显示,51眼中,视盘渗漏1眼,周边视网膜血管仍有渗漏者3眼,视网膜新生血管未退行1眼。结论VEP、F-ERG及FFA检查均可对眼挫伤患者进行有效的诊断及预后评估,临床应用中可根据患者情况联合或单独应用。

목적:평개안저혈관형광조영(FFA)재시망막좌상예후평개중적응용。방법선취시망막좌상환자51례위연구대상,분별채용시각유발전위(VEP)、FFA진행검사,병전부수방51례환자적시력변화。결과좌상적안정기VEP P100파진폭명현강저,잠복기명현연지;치료후VEP P100파진폭、잠복기균명현개선(P<0.05);FFA검사혼합성손상32안,단순성손상19안,기중38안표현위시망막진탕,14안표현위시망막출혈,5안표현위맥락막렬상,4안표현위황반렬공,8안표현위시신경좌상。치료후1개월복사안저혈관형광조영현시,51안중,시반삼루1안,주변시망막혈관잉유삼루자3안,시망막신생혈관미퇴행1안。결론VEP、F-ERG급FFA검사균가대안좌상환자진행유효적진단급예후평고,림상응용중가근거환자정황연합혹단독응용。
Objective To evaluate the application of retinal blood vessels fluorescence angiography( FFA) in the evaluation of prognosis of retinal contusion . Methods 51 patients with retinal contusion were selected as the research objects,who were respectively used visual evoked potential( the VEP), FFA for inspection,and all were followed up the changes in vision. Results The VEP amplitude of P100 waves of the eyes of the contusion was significantly low,the incubation period was significantly delayed;The VEP amplitude of P100 waves after treatment,the incubation period were both significantly improved(P < 0.05); 32 eyes of mixed injury were given FFA examination,19 eyes of simple injury,38 eyes among them showed commotio retinae,14 eyes showed retinal hemorrhage,five eyes showed choroid laceration,4 eyes showed macular hole,8 eyes showed optic nerve contusion. After treatment,fluorescence imaging showed review retinal blood vessels within 1 month,among 51 eyes,one eye was leakage of optic disk,three eyes were still leakage of peripheral retinal blood vessels,one eye was retinal neovascularization without regression. Conclusion The VEP,F- ERG and FFA examination can efficiently evaluate the diagnosis and prognosis to the patients of eye contusion,according to individual treatment schemes,they can be combined or applied independently by VEP、 F-ERG and FFA examinations.
