目的:探讨甲胎蛋白(AFP)与高尔基体蛋白73(GP73)对肝癌早期诊断的价值。方法采用酶联免疫法(Enzyme linked immunosorbent assay,ELISA)对肝癌患者80例、肝硬化患者30例血清中AFP、GP73进行检测,30例健康体检者作为对照。结果①肝癌组、肝硬化组、正常对照组AFP平均浓度为64.5μg/L、50.1μg/L、21.9μg/L (肝癌组vs对照组P<0.01,肝硬化组vs对照组P<0.01,肝硬化组vs肝癌组P<0.01),肝癌组、肝硬化组、正常对照组GP73平均浓度为268.4μg/L、192.3μg/L、49.8μg/L (P<0.01)。②AFP诊断肝癌的ROC曲线下面积为0.729,P=0.000,GP73诊断肝癌的ROC曲线下面积为0.892,P=0.000。AFP联合GP73对肝癌诊断的ROC曲线下面积为0.902。结论 GP73诊断肝癌的价值高于AFP,AFP+GP73联合检测不能提高诊断效率。

Objective To explore the value of alpha-fetoprotein (AFP) and Golgi protein 73 (GP73) in the early diagnosis of hepatocellular carcinoma. Methods AFP and GP73 were detected with enzyme-linked immunosor-bent assay (enzyme linked immunosorbent assay, ELISA) in 80 patients with hepatocellular carcinoma and 30 patients with hepatocirrhosis. 30 healthy subjects were selected as controls. Results ①The mean concentrations of AFP in hepatocellular carcinoma group, hepatocirrhosis group and control group was 64.5, 50.1, and 21.9 respectively, among which there were significantly differences (P<0.01). The concentrations of GP73 in hepatocellular carcinoma group, hepatocirrhosis group and control group was 268.4, 192.3 and 49.8 respectively, among which there were significantly differences (P<0.01).②The areas under ROC curves of AFP and GP73 were 0.729 and 0.892 respectively in diagno-sis of hepatocellular carcinoma. The area under ROC curve of AFP combined GP73 was 0.902 for diagnosis of hepato-cellular carcinoma. Conclusion Compared with AFP, GP73 is more sensitive and accurate in diagnosis of liver can-cer, and it is not necessary to test GP73 combined with AFP.
