目的:探讨跟腱损伤的诊断价值。方法:收治跟腱完全性撕裂和部分性撕裂患者16例,回顾性分析其MRI表现。结果:完全性撕裂11例,部分性撕裂5例。完全性撕裂显示跟腱连续性中断,断端分离,跟腱远端内见条片状等T1、稍长T2信号,肌腱断端交错呈“毛刷状”或回缩呈“杵状”改变,在T1WI、T2WI上呈混杂高信号。部分性跟腱撕裂表现跟腱局限性增粗,宽径/前后径比例减小,在T1WI、T2WI上肌腱内局限性高信号,肌束部分毛糙、不规则,但矢状位至少有1个层面是连续性的。结论:低场磁共振能清晰显示跟腱撕裂的位置、程度、周围软组织改变,以及是否合并骨髓水肿,对临床治疗有重要的指导意义。

목적:탐토근건손상적진단개치。방법:수치근건완전성시렬화부분성시렬환자16례,회고성분석기MRI표현。결과:완전성시렬11례,부분성시렬5례。완전성시렬현시근건련속성중단,단단분리,근건원단내견조편상등T1、초장T2신호,기건단단교착정“모쇄상”혹회축정“저상”개변,재T1WI、T2WI상정혼잡고신호。부분성근건시렬표현근건국한성증조,관경/전후경비례감소,재T1WI、T2WI상기건내국한성고신호,기속부분모조、불규칙,단시상위지소유1개층면시련속성적。결론:저장자공진능청석현시근건시렬적위치、정도、주위연조직개변,이급시부합병골수수종,대림상치료유중요적지도의의。
Objective:To investigate the diagnostic value of achilles tendon injury.Methods:16 cases with achilles tendon partial and completed tear were seleted.We retrospectively analyzed the MRI manifestations.Results:11 cases were complete tear;5 cases were partial tear.The complete tear of achilles tendon was discontinuous;end was separated;there were equal T1,long T2 signal of strip at the achilles tendon in the distal;there were "brush" or "clubbing" changes at the end of tendons;in T1WI,T2WI,it was mixed high signal.In the partial achilles tendon rupture patients,achilles tendon was localized thickening;width/diameter ratio was reduced;in T1WI,T2WI,they were the limitations of high signal in the tendon;partial fascicle was rough and irregular,but in the sagittal plane,at least 1 aspect was continuous.Conclusion:The low field MR can clearly show the achilles tendon rupture location, extent,change of the surrounding soft tissue,and with or without bone marrow edema.It has important instruction significance for clinical treatment.
