目的：探讨Solitaire AB支架半释放技术结合弹簧圈栓塞治疗前循环宽颈动脉瘤的有效性和安全性。方法对2011年1月～2013年10月33例患者34个前循环宽颈动脉瘤采用Solitaire AB支架辅助弹簧圈栓塞，回顾性分析其临床资料和疗效。结果所有患者均采用支架半释放技术，33例患者共应用Solitaire AB支架34个，均成功到位并准确释放。1个支架内血栓形成，其余无手术相关并发症。动脉瘤完全栓塞26个，瘤颈残留6个，部分栓塞2个。载瘤动脉通畅。除1例后交通动脉瘤病人术后3 d并发颅内出血死亡外，余32例临床随访6～12个月无临床症状加重，未发生再出血及缺血并发症。22例术后6～12个月行DSA随访，2个动脉瘤瘤颈有残留，未见支架内狭窄和闭塞。结论 Solitaire AB支架操作简便，采用支架半释放技术结合弹簧圈栓塞治疗前循环宽颈动脉瘤是安全的和有效的。
목적：탐토Solitaire AB지가반석방기술결합탄황권전새치료전순배관경동맥류적유효성화안전성。방법대2011년1월～2013년10월33례환자34개전순배관경동맥류채용Solitaire AB지가보조탄황권전새，회고성분석기림상자료화료효。결과소유환자균채용지가반석방기술，33례환자공응용Solitaire AB지가34개，균성공도위병준학석방。1개지가내혈전형성，기여무수술상관병발증。동맥류완전전새26개，류경잔류6개，부분전새2개。재류동맥통창。제1례후교통동맥류병인술후3 d병발로내출혈사망외，여32례림상수방6～12개월무림상증상가중，미발생재출혈급결혈병발증。22례술후6～12개월행DSA수방，2개동맥류류경유잔류，미견지가내협착화폐새。결론 Solitaire AB지가조작간편，채용지가반석방기술결합탄황권전새치료전순배관경동맥류시안전적화유효적。
Objective To evaluate the efficiency and safety of Solitaire AB coil embolization of wide-necked anterior circulation aneurysm using a semi-deployment technique.Methods From January 2011 to October 2013,34 wide-necked anterior circulation aneurysms in 33 patients were treated with stent-assisted coil embolization at our institution.The clinical records and treatment efficacy were retrospectively analyzed.Results Thirty-four stents were semi-deployed successfully on 33 patients in the desired locations of the aneurysms.In-stent thrombosis occurred in 1 patient.The aneurysms were occluded totally in 26,subtotally in 6 and partially in 2.The entire parent arteries were patent.One patient with posterior communicating artery aneurysm died from intracranial hemorrhage 3 days after the procedure.During the 6-12 month follow-up, there was no exacerbation of symptoms or newly-developed bleeding or infarction in the remaining 32 patients.Follow-up digital subtraction angiography showed no in-stent stenosis or occlusion in 22 patients and neck remnants in 2 patients.Conclusion Semi-deployed Solitaire AB coil embolization is technically easy,safe and effective in the treatment of wide-necked anterior circulation aneurysm.
