目的:探讨表观扩散系数(ADC)直方图对骨肉瘤新辅助化疗疗效评价的价值。方法对10例经病理证实的四肢骨肉瘤在新辅助化疗前3 d内、化疗后术前3 d内进行常规MRI和DWI扫描,采用MR设备自带后处理软件生成ADC图,将新辅助化疗前、后的整个瘤体的ADC图导入基于Matlab的直方图分析软件获得ADC值分布直方图。将10例肿瘤按手术病理Huvos分级法分为组织学反应良好组和组织学反应较差组,比较两组新辅助化疗前、后ADC直方图和平均ADC值的变化。结果骨肉瘤新辅助化疗后组织学反应良好组(6例)ADC直方图平均值右移,而组织学反应较差组(4例)ADC直方图显示平均值右移不明显甚至左移;两组间平均ADC值变化率的差异有统计学意义(P值=0.014)。结论 ADC直方图有助于评价骨肉瘤新辅助化疗的疗效。

[Abstraut] Objective To investigate the efficacy of apparent diffusion coefficient(ADC)histogram for assessing neoadjuvant chemotherapeutic response of osteosarcoma.Methods Conventional and diffusion-weighted MR imaging was performed on 10 patients with pathologically confirmed limb osteosarcoma within 3 days before pre-operative neoadjuvant chemotherapy and within 3 days before surgery.ADC maps before and after neoadjuvant chemotherapy were automatically generated at the workstation using the vendor software and ADC histograms were obtained using a Matlab-based customized software.The patients were divided into two groups with good and poor chemotherapy responses according to the postoperative Huvos pathological grading. The change of pre-and post-chemotherapy ADC histograms and mean ADC values between the two groups were compared.Results The mean values of the ADC histograms were shifted to the right in the group with good response(6 patients)and unchanged or shifted to the left in the group with poor response(4 patients).The change of pre-and post-neoadjuvant chemotherapy mean ADC values was statistically different between the two groups(P=0.014).Conclusion The ADC histogram is helpful for evaluating neoadjuvant chemotherapeutic response of osteosarcoma.
