目的:探讨MMP-9基因-1562和R279Q位点多态性与儿童呼吸道合胞病毒(respiratory syncytial virus,RSV)毛细支气管炎易感性及严重程度之间的关系。方法对RSV感染致毛细支气管炎患儿142例(实验组)和健康儿童80例(对照组),采用聚合酶链反应-限制性片段长度多态性(PCR-RFLP)分析方法,检测MMP-9基因-1562和R279Q位点多态性基因型,比较不同位点不同基因型与毛细支气管炎易感性及严重程度之间的关系。结果 MMP-9基因-1562位点存在3个基因型,即C/C、C/T、T/T,分布频率分别为72.54%、26.01%、1.41%;MMP-9基因R279Q位点存在3个基因型,即C/C、C/G、G/G,分布频率分别为58.45%、33.80%、7.75%。MMP-9基因-1562位点不同基因型之间,毛细支气管炎严重程度差异有统计学意义(P<0.05)。MMP-9-1562位点的C/T基因型携带者比C/C基因型携带者更容易发生毛细支气管炎,风险上升2.25倍(95%C. I.:1.08~4.72);MMP-9 R279Q位点的C/C基因型携带者更容易发生毛细支气管炎,风险上升1.90倍(95%C.I.:1.03~3.58)。结论 MMP-9基因-1562和R279Q位点多态性基因型与RSV毛细支气管炎易感性相关,而-1562位点多态性与其严重程度相关。

목적:탐토MMP-9기인-1562화R279Q위점다태성여인동호흡도합포병독(respiratory syncytial virus,RSV)모세지기관염역감성급엄중정도지간적관계。방법대RSV감염치모세지기관염환인142례(실험조)화건강인동80례(대조조),채용취합매련반응-한제성편단장도다태성(PCR-RFLP)분석방법,검측MMP-9기인-1562화R279Q위점다태성기인형,비교불동위점불동기인형여모세지기관염역감성급엄중정도지간적관계。결과 MMP-9기인-1562위점존재3개기인형,즉C/C、C/T、T/T,분포빈솔분별위72.54%、26.01%、1.41%;MMP-9기인R279Q위점존재3개기인형,즉C/C、C/G、G/G,분포빈솔분별위58.45%、33.80%、7.75%。MMP-9기인-1562위점불동기인형지간,모세지기관염엄중정도차이유통계학의의(P<0.05)。MMP-9-1562위점적C/T기인형휴대자비C/C기인형휴대자경용역발생모세지기관염,풍험상승2.25배(95%C. ObjectiveTo explore the relationship between MMP-9 -1562 and R279Q polymorphism genotype and susceptibility to respiratory syncytial virus(RSV)bronchiolitis and the severity of RSV bronchiolitis.Methods142 children of RSV bronchiolitis selected from February 2011 to March 2013 and seemed as experimental group. And 80 normal children were selected as control group. Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)analysis was performed to detect MMP-9 -1562 and R279Q polymorphism genotype.ResultsMMP-9 -1562 presence of three genotypes, namely C/C, C/T, T/T, frequency distribution was 72.54%, 26.01%, 1.41%, respectively. MMP-9 R279Q presence of three genotypes, namely C/C, C/G, G/G, frequency distribution was 58.45%, 33.80%, 7.75%, respectively. Bronchiolitis severity in different genotypes of MMP-9 -1562 site had statistically significant differences(P<0.05). MMP-9 -1562 C/T genotype easier susceptibility to respiratory syncytial virus (RSV)bronchiolitis than the C/C genotype, the risk may be increased by 2.25(95%C.I.: 1.08-4.72)times. Gene MMP-9 R279Q C/C genotype easier susceptibility to respiratory syncytial virus(RSV)bronchiolitis, the risk may rise 1.90(95%C.I.:1.03-3.58) times.ConclusionMMP-9 -1562 and R279Q polymorphism genotype were related to susceptibility of respiratory syncytial virus (RSV)bronchiolitis, while -1562 polymorphism genotype was related to the severity of RSV bronchiolitis.
