目的:通过与传统的比例法在新发涂阳病人中对结核分枝杆菌进行耐药检测比较,评价 HAIN(线性探针)用于检测原发性耐药结核的可行性。方法随机抽取我院2012年1月至2013年3月新发涂阳病人的痰标本,分别进行 HAIN 检测和酸性罗氏培养、比例法药敏试验,并对结果进行分析比较,共323例病人被纳入分析。结果以比例法为金标准,HAIN 方法对异烟肼(P =0.823)和利福平(P =1.000)的耐药检测结果与金标准的差异无统计学意义,HAIN 检测异烟肼耐药的灵敏性为80.4%、特异性为96.6%,检测利福平耐药的灵敏性为95.8%、99.6%。HAIN 对原发耐多药结核的检测(P =0.790)与金标准的差异也无统计学意义,灵敏性为78.9%、特异性为97.9%。结论HAIN 技术适用于临床原发耐药结核的快速筛查。

Objective To evaluate the rapid drug resistance detection of Mycobacterium tuberculosis by HAIN(line probe assay),we also did drug resistance tests using proportion method and analyzed their characteristics.Methods we selected new smear-positive TB cases from 2012 January to 2013 March in our hospital,,isolated strains from clinical specimens,used HAIN to find the strains which were drug resistant to INH、RIF,and the results were compared with proportion method ,323 cases were enrolled in the analysis.Results The proportion method was used as golden stand-ard,There was no significant difference on INH(P =0.823)and on RIF(P =1.000)between HAIN and gold standard. the sensitivity and specificity of HAIN in detecting INH resistant cases was 80.4% and 96.6% respectively,the sensi-tivity and specificity of HAIN in detecting RIF resistant cases was 95.8% and 99.6% respectively.There was no sig-nificant difference in detecting new MDR cases between HAIN and gold standard,the sensitivity and specificity of HAIN was 79.5% and 97.9%.Conclusion the HAIN technique was suitable as a first-line screening assay for new drug resistant TB.
