目的:探讨在全麻并喉罩通气情况下,经支气管镜下高频电刀治疗气管及主支气管内新生物阻塞的疗效和安全性。方法选择因气管、主支气管腔内良、恶性肿瘤而阻塞大气道导致呼吸困难的患者32例。通过异丙酚、芬太尼和维库溴铵对患者进行静脉全身麻醉,用喉罩通气保护,经支气管镜用高频电刀对病变部位进行治疗。结果32例患者经支气管镜下高频电力治疗后,均感到呼吸困难症状得到明显改善。32例患者治疗前后FEV1分别为(1.62±0.24)L和(2.48±0.46)L(P﹤0.01);气道直径分别为(3.15±1.41) mm和(8.12±1.20)mm(P﹤0.01);气促评分分别为(1.95±0.69)级和(0.73±0.34)级(P﹤0.01)。结论在全麻经喉罩通气情况下,用高频电刀治疗气管和主支气管新生物气道阻塞,能明显解除气道阻塞,改善患者呼吸困难症状,且安全可行。

목적:탐토재전마병후조통기정황하,경지기관경하고빈전도치료기관급주지기관내신생물조새적료효화안전성。방법선택인기관、주지기관강내량、악성종류이조새대기도도치호흡곤난적환자32례。통과이병분、분태니화유고추안대환자진행정맥전신마취,용후조통기보호,경지기관경용고빈전도대병변부위진행치료。결과32례환자경지기관경하고빈전력치료후,균감도호흡곤난증상득도명현개선。32례환자치료전후FEV1분별위(1.62±0.24)L화(2.48±0.46)L(P﹤0.01);기도직경분별위(3.15±1.41) mm화(8.12±1.20)mm(P﹤0.01);기촉평분분별위(1.95±0.69)급화(0.73±0.34)급(P﹤0.01)。결론재전마경후조통기정황하,용고빈전도치료기관화주지기관신생물기도조새,능명현해제기도조새,개선환자호흡곤난증상,차안전가행。
Objective To explore the therapeutic effects and safety of high-frequency electrocautery via bron-choscope in the treatment of neoplasm obstruction in trachea and main bronchus under general anesthesia and larynge-al mask airway. Methods Thirty-two patients with dyspnea caused by benign and malignant stenosis in trachea as well as main bronchus were collected. High-frequency electrocautery via bronchoscope was used under general anes-thesia through propofol,fentanyl and vecuronium in the protection of laryngeal mask airway. Results Dyspnea was re-lieved obviously after the treatment of high-frequency electrocautery via bronchoscope in all 32 patients. FEV1 was (1.62±0.24)Land(2.48±0.46)L(P﹤0.01),airwaydiameterwas(3.15±1.41)mmand(8.12±1.20)mm (P﹤0. 01),dyspnea index was(1. 95 ± 0. 69)and(0. 73 ± 0. 34)(P﹤0. 01)before and after treatment. Conclusion High-frequency electrocautery via bronchoscope under general anesthesia and laryngeal mask airway can relieve neo-plasm obstruction and dyspnea obviously,which is safe and feasible.
