目的 观察在经颅多普勒超声引导下行颈内静脉导管置入术术中及术后并发症的发生率,探讨经颅多普勒超声的应用价值.方法 选取2010年9月至2011年9月于郑州市第七人民医院住院的慢性肾功能不全患者70例,其中男38例,女32例,平均年龄(46±8)岁.采用数字表法随机分为直接穿刺组和经颅多普勒超声组,每组各35例,分别实施临时性颈内静脉导管置入手术,观察不同手术方案下各组术中及术后并发症的发生率.采用分层资料卡方检验进行数据统计.结果 经颅多普勒超声组误穿动脉(2.9% vs 0)、穿刺失败(8.6% vs 0)的发生率显著低于直接穿刺组,两组术后渗血发生率(5.7%vs 2.9%)差异无统计学意义.结论 经颅多普勒超声引导下行颈内静脉导管置入术成功率明显提高,误穿动脉的发生率降低,有一定的应用前景.

Objective To evaluate the role of transcranial doppler in the process of tibial vein catheterization.Methods Seventy patients with chronic kidney disease (CKD) stage 5 in need of tibial vein catheterization were enroilled in this investigation.The patients were assigned to group A (non-device) and group B (under transcranial doppler).The medical records of eligible patients were reviewed with regard to the complications including hematoma,oozing and etc during the operation.Results Group B showed lower misparacentesis incidence (0) and lower failure rate in comparision with group A (2.9% and 8.6%,respectively).The placement of tibial vein catheters was performed without difficulty when the site of placement was chosen on the basis of transcranial doppler.Conclusions Transcranial doppler may be helpful in locating central venous during the placement of tibial vein catheters.
