目的:探讨新生儿低血糖脑损伤的临床、头颅磁共振( MRI )的特征。方法选择新生儿低血糖脑损伤的病例42例(脑损伤组)及同期住院的新生儿低血糖无脑损伤的病例45例(无脑损伤组),对两组患儿的临床资料进行回顾性分析。结果脑损伤组平均最低血糖值明显低于无脑损伤组(P<0.01),低血糖持续时间明显长于无脑损伤组(P<0.01),脑损伤组治疗24 h后血糖恢复正常者明显低于无脑损伤组(P<0.01)。脑损伤组头颅MRI早期表现为受损部位的弥散加权成像技术(DWI)高信号42例(100%),脑损伤的部位多分布在顶枕叶38例(90.5%)。脑损伤组30例患儿进行6~18个月随访,发生低血糖脑损伤神经系统后遗症20例(66.7%);无脑损伤组无神经系统后遗症。结论新生儿血糖值越低,低血糖持续时间越久,脑损伤越重,低血糖脑损伤预后差,神经系统后遗症发生率高。

목적:탐토신생인저혈당뇌손상적림상、두로자공진( MRI )적특정。방법선택신생인저혈당뇌손상적병례42례(뇌손상조)급동기주원적신생인저혈당무뇌손상적병례45례(무뇌손상조),대량조환인적림상자료진행회고성분석。결과뇌손상조평균최저혈당치명현저우무뇌손상조(P<0.01),저혈당지속시간명현장우무뇌손상조(P<0.01),뇌손상조치료24 h후혈당회복정상자명현저우무뇌손상조(P<0.01)。뇌손상조두로MRI조기표현위수손부위적미산가권성상기술(DWI)고신호42례(100%),뇌손상적부위다분포재정침협38례(90.5%)。뇌손상조30례환인진행6~18개월수방,발생저혈당뇌손상신경계통후유증20례(66.7%);무뇌손상조무신경계통후유증。결론신생인혈당치월저,저혈당지속시간월구,뇌손상월중,저혈당뇌손상예후차,신경계통후유증발생솔고。
Objective To explore the clinical and magnetic resonance imaging ( MRI) characteristics and prognosis of neonatal hy-poglycemia brain injury .Methods Forty-two infants with neonatal hypoglycemia brain injury and 45 infants with non neonatal hypoglycemia brain injury were chosen from Jun 2009 to Jun 2013 in department of neonatology in Anhui Provincial Children′s Hospital,and the clinical da-ta were analyzed .Results The lowest blood glucose levels were lower and the duration of hypoglycemia was longer in brain injury group than those of non brain injury group,and the difference was statistically significant (P<0.01).The rate of blood glucose levels back up to normal in brain injury group were lower than those of non brain injury group 24 h after treatment(P<0.01).All infants in brain injury group had brain injury by MRI diffusion weighted imaging (DWI),38 cases with parietal and occipital lobe injury .After 6 months to 18 months follow-up,20 cases in brain injury group (30 cases) had neurological sequelae ,and non brain injury group had favorable prognosis .Conclusion The lower of blood glucose levels and longer of duration of hypoglycemia ,the heavier of brain injury .Neonate with hypoglycemic brain damage has poor prognosis and high incidence of neurological sequelae .
