目的:研究支气管扩张症患者下呼吸道细菌定植(LABC)及其与气道阻塞的关系。方法入选明确诊断的支气管扩张症患者40例,于稳定期进行痰细菌定量培养、肺功能检查。结果在40例稳定期支气管扩张症患者中 LABC 阳性组15例,阴性组25例,提示存在细菌定植,以铜绿假单胞菌为主,并发现 LABC 阳性组第1秒用力呼气量(FEV1)低于 LABC 阴性组(P ﹤0.05),用力肺活量(FVC)及 FEV1/ FVC 两组比较差异无统计学意义(P ﹥0.05)。结论稳定期支气管扩张症患者存在下呼吸道细菌定植,定植菌以铜绿假单胞菌为主,肺功能损害严重者更容易出现细菌定植,细菌定植可进一步加重气道阻塞。

목적:연구지기관확장증환자하호흡도세균정식(LABC)급기여기도조새적관계。방법입선명학진단적지기관확장증환자40례,우은정기진행담세균정량배양、폐공능검사。결과재40례은정기지기관확장증환자중 LABC 양성조15례,음성조25례,제시존재세균정식,이동록가단포균위주,병발현 LABC 양성조제1초용력호기량(FEV1)저우 LABC 음성조(P ﹤0.05),용력폐활량(FVC)급 FEV1/ FVC 량조비교차이무통계학의의(P ﹥0.05)。결론은정기지기관확장증환자존재하호흡도세균정식,정식균이동록가단포균위주,폐공능손해엄중자경용역출현세균정식,세균정식가진일보가중기도조새。
Objective To investigate the relationship between lower airway bacterial colonization (LABC)existed in stable phase bronchiectasis and to establish the relationship between LABC and air-way obstruction in patients with bronchiectasis. Methods A total of 40 patients with sever bronchiectasis in stable phase were included in this study,sputum samples were analyzed for bacterial quantitative cul-ture. Pulmonary function test was evaluated at the same time. Results At the stable phase,fifteen pa-tients were considered LABC colonized. Pseudomonas aeruginosa influenzae was the most important colo-nized pathogen. And the patients’FEV1 with LABC was lower than that without LABC(P ﹤ 0. 05),the difference of FVC and FEV1 / FVC between the two groups had no statistical significance(P ﹥ 0. 05). Conclusions Some patients with clinically stable bronchiectasis has LABC,the lung function damage more serious,the bacterial colonization easier,and it can further aggravate airway obstruction.
