目的：分析老年急性上消化道出血（ acute upper gastrointestinal bleeding，AUGIB）患者的临床特点。方法对2006年1月至2012年12月住院诊治的老年AUGIB患者的临床资料进行回顾性分析，并选择中青年AUGIB患者作为对照组进行对比。结果共纳入258例老年AUGIB患者，其中消化性溃疡108例（41.9%），胃癌56例（21.7%），急性胃黏膜病变40例（15.5%），食管胃底静脉曲张32例（12.4%），其他22例（8.5%）。老年AUGIB患者胃癌比率、伴随疾病比率及服用阿司匹林用药比率均高于中青年组，且再出血率及病死率亦高于中青年组。结论老年AUGIB患者仍以消化性溃疡为主要病因，具有恶性肿瘤比率高、伴发疾病率较高、服用阿司匹林类药物比率高、临床预后差等特点，应重视AUGIB的老年患者的诊治。
목적：분석노년급성상소화도출혈（ acute upper gastrointestinal bleeding，AUGIB）환자적림상특점。방법대2006년1월지2012년12월주원진치적노년AUGIB환자적림상자료진행회고성분석，병선택중청년AUGIB환자작위대조조진행대비。결과공납입258례노년AUGIB환자，기중소화성궤양108례（41.9%），위암56례（21.7%），급성위점막병변40례（15.5%），식관위저정맥곡장32례（12.4%），기타22례（8.5%）。노년AUGIB환자위암비솔、반수질병비솔급복용아사필림용약비솔균고우중청년조，차재출혈솔급병사솔역고우중청년조。결론노년AUGIB환자잉이소화성궤양위주요병인，구유악성종류비솔고、반발질병솔교고、복용아사필림류약물비솔고、림상예후차등특점，응중시AUGIB적노년환자적진치。
Objective To analyze the clinical features of elder patients with acute upper gastrointestinal bleeding (AUGIB). Methods From January 2006 to December 2012,the clinical data of elder patients with AUGIB were retro-spectively analyzed. Moreover,260 young and middle-aged patients with AUGIB were enrolled as the control group. Re-sults A total of 258 elder patients were enrolled in this study,including 108 cases of peptic ulcer(41. 9%),56 cases of gastric cancer(21. 7%),40 cases of acute gastric mucosal lesions(15. 5%),32 cases of esophageal and gastric varices (12. 4%)and others(8. 5%). The rates of gastric cancer,aspirin using,underlying diseases,re-bleeding and mortality in elder patients were higher than those in the control group. Conclusions Peptic ulcer is the main reason of AUGIB in elderly patients. AUGIB in elderly patients has high rates of malignant tumors,complicated diseases,aspirin medication and poor clinical prognosis,so the diagnosis and treatment should be paid attention to.
