目的:探讨不同剂量rt-PA静脉溶栓治疗急性脑梗死的疗效。方法随机选取我院2010-02-2012-11收治的急性脑梗死患者82例为研究对象,分为小剂量(0.6 mg/kg)静脉溶栓A组(41例),大剂量(0.9 mg/kg)B组(41例),采用神经功能缺损评分(NIHSS)评价2组疗效,对比并发症发生率。结果2组NIHSS评分均优于治疗前(P<0.05),但治疗后各阶段2组无显著差异(P>0.05);不良反应发生率B组明显高于A组(P<0.05)。结论 rt-PA静脉溶栓可显著改善急性脑梗死患者预后,但治疗效果与剂量大小关联性不显著,大剂量rt-PA会增加出血风险。

목적:탐토불동제량rt-PA정맥용전치료급성뇌경사적료효。방법수궤선취아원2010-02-2012-11수치적급성뇌경사환자82례위연구대상,분위소제량(0.6 mg/kg)정맥용전A조(41례),대제량(0.9 mg/kg)B조(41례),채용신경공능결손평분(NIHSS)평개2조료효,대비병발증발생솔。결과2조NIHSS평분균우우치료전(P<0.05),단치료후각계단2조무현저차이(P>0.05);불량반응발생솔B조명현고우A조(P<0.05)。결론 rt-PA정맥용전가현저개선급성뇌경사환자예후,단치료효과여제량대소관련성불현저,대제량rt-PA회증가출혈풍험。
Objective To investigate the the curative effect of different doses of rt-PA intravenous thrombolysis treating patients with acute cerebral infarction.Methods 82 patients with acute cerebral infarction in our hospital from February 2010 to November 2012 were randomly divided into the intravenous thrombolysis group A (n= 41 ,0 .6 mg/kg ) and the intravenous thrombolysis group B (n= 41 ,0 .9 mg/kg).The neural function defect score (NIHSS) and complications incidence of two groups were compared.Results The NIHSS of two groups after treatment were more than these before treatment ,but had no significant difference in different stages of the two groups after treatment. The incidence of adverse reactions in group B was higher than that in group A.Conclusion Rt-PA intravenous thrombolysis can significantly improve the prognosis of patients with acute cerebral infarction ,but no significant correlation between therapeutic effect and dose size.Large dose of rt-PA can increase the bleeding risk .
