目的:比较两种代谢综合征(MS)诊断标准在北京某村镇中年农民人群中应用的差异。方法采用中华医学会糖尿病分会(CDS)和经美国心脏协会修订的美国国家胆固醇教育计划(NCEP)成人治疗组第三次报告(ATPⅢ)的两种诊断标准,调查632例中年农民MS的患病情况,并比较两种标准的诊断符合率。结果应用CDS及AIPⅢ诊断标准,结果显示,MS患病率分别为17.0%(108/632)、20.7%(131/632),差异有高度统计学意义(P<0.01)。 CDS与ATPⅢ诊断标准的诊断符合率为88.4%(Kappa=0.887,P<0.01)。结论两种标准在MS的诊断结果上虽存在差异,但CDS与ATPⅢ诊断符合率的一致性较高。

Objective To compare the application comparison of two diagnostic criteria for metabolic syndrome (MS) among the middle aged farmers in a village of Beijing. Methods 632 middle aged farmers in a village were diagnosed for MS by the Chinese Medical Association Diabetes Society (CDS) and third report of National Cholesterol Education Prongram-Adult Treatment Panel (ATPⅢ) criteria. The diagnosis accordance rates were compared among the different diagnostic criteria. Results The MS prevalence rates in the middle aged farmers were 17.0% (108/632), 20.7% (131/632), according to CDS and ATPⅢ criteria, and there were difference among the criteria (P<0.01). The coincidences in the diagnosis of MS were 88.4% (Kappa=0.887, P<0.01) using CDS and ATPⅢ. Conclusion There are significant differences from these two criteria to diagnose MS, but the agreement in the diagnosis of MS using CDS and ATPⅢdefinition is higher.
