目的:测定类风湿关节炎(RA)患者血清中IL-22的水平并分析其临床意义。方法采用ELISA法检测70例初诊的RA患者(RA组)和18例健康体检者(健康对照组)血清中IL-22的水平,其中RA组患者进一步分为RA活动组和RA非活动组,有骨侵蚀组和无骨侵蚀组,类风湿因子(RF)阳性组和RF阴性组,抗环瓜氨酸肽(抗CCP)抗体阳性组和抗CCP抗体阴性组。分析IL-22在不同组别的表达及其与RA患者临床及炎性指标的相关性。结果①RA组患者血清IL-22水平[(80.30±37.73)pg/mL]较健康对照组[(29.29±7.38)pg/mL]明显升高,差异有高度统计学意义(P<0.01);其中RA活动组[(86.58±41.21)pg/mL]与RA非活动组[(63.24±17.98)pg/mL]及健康对照组[(29.29±7.38)pg/mL]比较,差异均有统计学意义(P<0.05);RA组患者有骨侵蚀组[(94.71±42.20)pg/mL]与RA无骨侵蚀组[(56.03±18.16)pg/mL]比较,差异有统计学意义(P<0.05);②IL-22在RF阳性组、抗CCP抗体阳性组[(84.65±38.47)、(100.3±43.68)pg/mL]明显高于RF阴性组及抗CCP抗体阴性组[(59.48±39.01)、(64.12±18.32)pg/mL],差异均有统计学意义(P<0.05);③RA组患者血清IL-22中水平与DAS28、RF呈正相关性(r =0.2883,P=0.0382;r=0.3670,P=0.0074)。结论 RA患者血清IL-22水平明显升高,可能参与了RA的发病机制,可作为RA病情活动及骨侵蚀的预测指标。

Objective To test the serum level of IL-22 in patients with rheumatoid arthritis(RA) and analyze its clinical significance. Methods Concentrations of IL-22 in the serum of 70 newly diagnosed RA patients (RA group) and 18 healthy controls (healthy control group) were measured by enzyme-linked immunosorbent assay. Patients of RA group were further divided into active RA group and non-active RA group, bone erosions group and without bone erosions group, rheumatoid factor (RF) positivity group and RF negative group, anti-cyclic citrullinated peptide antibodies (anti-CCP) positive group and anti-CCP negative group. The expression of IL-22 in different groups and the correlation be-tween IL-22 and the clinical inflammatory indexes of RA patients were analyzed. Results ①Serum IL-22 content in RA group [(80.30±37.73) pg/mL] were significantly higher than that of healthy control group [(29.29±7.38) pg/mL], the difference was statistically significant (P< 0.01). There was a significant difference between active RA group [(86.58±41.21) pg/mL], non-active RA group [(80.30±37.73) pg/mL] and healthy control group [(29.29±7.38) pg/mL], the differ-ences were statistically significant (P< 0.05). There was a significant difference between bone erosions group [(94.71±42.20) pg/mL] and without bone erosions group [(56.03±18.16) pg/mL] (P<0.05);②IL-22 in the RF positive group and anti-CCP antibody positive group [(84.65±38.47), (100.3±43.68) pg/mL] were higher than those of RF negative groups and anti-CCP antibody negative group [(59.48±39.01), (64.12±18.32) pg/mL], the differences were statistically significant (P<0.05);③the levels of serum IL-22 in RA patients were positive related with disease activity index DAS28 and RF (r=0.2883, P=0.0382;r=0.3670, P=0.0074). Conclusion The significantly elevated IL-22 in the serum of patients with es-tablished RA indicates that IL-22 may be involved in the pathogenesis of RA, and can be used as a predictor for the disease activity and bone erosion of RA.
