目的：探讨帕罗西汀联合中药天王补心汤治疗伴有焦虑症状抑郁症的临床效果及安全性。方法将98例伴有焦虑症状抑郁症患者随机分为研究组(n=49例)和对照组(n=49例)，研究组给予帕罗西汀联合天王补心汤治疗，对照组给予帕罗西汀治疗，治疗6周。采用汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)及副反应量表(TESS)评定药物的疗效及不良反应。结果研究组和对照组的有效率分别为91.8%和77.6%，两组比较差异有统计学意义(P<0.05)；两组治疗后HAMD和HAMA评分较治疗前均显著下降(P<0.01)；研究组在治疗2、4、6周末HAMD和HAMA评分下降较对照组显著(P<0.05)；研究组发生口干、恶心和失眠的不良反应明显少于对照组(P<0.05)。结论帕罗西汀联合天王补心汤治疗伴有焦虑症状的抑郁症的效果比单用帕罗西汀更显著，能较快缓解患者的抑郁、焦虑症状，且不良反应少，安全性高。
목적：탐토파라서정연합중약천왕보심탕치료반유초필증상억욱증적림상효과급안전성。방법장98례반유초필증상억욱증환자수궤분위연구조(n=49례)화대조조(n=49례)，연구조급여파라서정연합천왕보심탕치료，대조조급여파라서정치료，치료6주。채용한밀이돈억욱량표(HAMD)、한밀이돈초필량표(HAMA)급부반응량표(TESS)평정약물적료효급불량반응。결과연구조화대조조적유효솔분별위91.8%화77.6%，량조비교차이유통계학의의(P<0.05)；량조치료후HAMD화HAMA평분교치료전균현저하강(P<0.01)；연구조재치료2、4、6주말HAMD화HAMA평분하강교대조조현저(P<0.05)；연구조발생구간、악심화실면적불량반응명현소우대조조(P<0.05)。결론파라서정연합천왕보심탕치료반유초필증상적억욱증적효과비단용파라서정경현저，능교쾌완해환자적억욱、초필증상，차불량반응소，안전성고。
Objective To dicuss the efficacy and safety of paroxetine combined with Tianwangbuxin decoction in treat-ment of depression associated with anxiety. Methods 98 depression patients with anxiety symptoms were randomly di-vided into research group (n=49) and control group (n=49) for 6 weeks.The research group was given paroxetine com-bined with Tianwangbuxin decoction treatment,the control group was given paroxetine theratment,treatment of 6 weeks. The drug efficacy and adverse reactions were evaluated by Hamilton depression scale (HAMD),Hamilton anxiety scale (HAMA) and treatment emergent symptom scaie (TESS). Results The effective rate of the research group and control group was 91.8%and 77.6%respectively,there was significant difference between the two groups (P<0.05).The scores of HAMD and HAMA in both group were significantly decreased after treatment than before (P<0.01).The scores of HAMD and HAMA in research group were significantly decreased than the scores in control group after treatment at the end of 2,4,6 weeks (P<0.05).The adverse reaction of dry mouth,nausea and insomnia in research group were signifi-cantly less than those of the control group (P<0.05). Conclusion It is more effective of paroxetine combined with Tian-wangbuxin decoction in treatment of depression associated with anxiety than of simple paroxetine,and it can quickly re-lieve depression and anxiety symptoms,there is less adverse reaction and high safety.
