目的：观察低分子肝素、胰岛素联合血浆置换治疗急性高脂血症胰腺炎的临床价值。方法选择本院收治的84例急性高脂血症胰腺炎患者，按照随机分组原则分为对照组与观察组，每组42例。对照组应用低分子肝素、胰岛素联合治疗，观察组在对照组治疗的基础上应用血浆置换治疗，比较两组的治疗效果，观察治疗前后肿瘤坏死因子-α(栽晕云-α)、血清淀粉酶及三酰甘油(栽G)水平的变化。结果观察组总有效率显著高于对照组，差异有统计意义(孕<0.05)。两组治疗前栽晕云-α、血清淀粉酶及栽G水平差异无统计学意义(P>0.05)，治疗后两组栽晕云-α、血清淀粉酶及栽G水平均显著改善(P<0.05)，且观察组改善情况显著优于对照组(P<0.05)。结论低分子肝素、胰岛素联合血浆置换治疗急性高脂血症胰腺炎效果明显，值得临床推广应用。

목적：관찰저분자간소、이도소연합혈장치환치료급성고지혈증이선염적림상개치。방법선택본원수치적84례급성고지혈증이선염환자，안조수궤분조원칙분위대조조여관찰조，매조42례。대조조응용저분자간소、이도소연합치료，관찰조재대조조치료적기출상응용혈장치환치료，비교량조적치료효과，관찰치료전후종류배사인자-α(재훈운-α)、혈청정분매급삼선감유(재G)수평적변화。결과관찰조총유효솔현저고우대조조，차이유통계의의(잉<0.05)。량조치료전재훈운-α、혈청정분매급재G수평차이무통계학의의(P>0.05)，치료후량조재훈운-α、혈청정분매급재G수평균현저개선(P<0.05)，차관찰조개선정황현저우우대조조(P<0.05)。결론저분자간소、이도소연합혈장치환치료급성고지혈증이선염효과명현，치득림상추엄응용。
Objective To observe the clinical value of low molecular heparin,insulin and plasma exchange in the treat-ment of acute hyperlipidemic pancreatitis (HLP). Methods Eighty-four patients with acute HLP admitted into our hos-pital were evenly divided into control group and observation group in random.Low molecular heparin and insulin was given to the control group,while plasma exchange was added to the observation group on the basis of the control group. The therapeutic effect and levels of tumor necrosis factor-α (TNF-α),serum amylase,triglyceride (TG) before and after treatment were compared. Results The total effective rate in the observation group was higher than that in the control group,the difference was significant (P>0.05).Before treatment,there was no significant difference about the level of TNF-α,serum amylase and TG in both groups (P>0.05).After treatment,these three indexes in the two groups were im-proved (P<0.05),and the improvements in the observation group were superior to those in the control group (P<0.05). Conclusion In the treatment of acute HLP,low molecular heparin,insulin and plasma exchange can obtain a remarkable effect,which is worthy of expansion and application in clinic.
