目的:探讨COH过程中主导卵泡生长速度对妊娠结局的影响。方法选取黄体中期长方案60例患者,其中卵泡≥14mm后继续Gn,天数≥4天为研究组,另卵泡≥14mm后继续Gn<4天为对照组,对两组的年龄、不孕年限、Gn天数、获卵数、受精率、2PN率、卵裂率、优胚率、妊娠率进行分析。结果1.两组平均年龄(31.57±2.73)岁,不孕年限(3.32±1.20)年,Gn天数(10.21±1.22)天,p>0.05,差异均无统计学意义.两组的获卵数(9.50±1.32)与(9.27±1.92),受精率(82.40±13.13)%与(71.68±21.57)%,,2PN率(93.03±10.62)%与(94.91±10.49)%,卵裂率(75.61±12.10)%与(70.82±21.95)%,差异也无统计学意义(P>0.05),而研究组于对照组的优胚率(84.75±14.68)%与(55.45±20.18)%,妊娠率(50%)与(20%)差异有统计学意义(P<0.05)。结论在COH过程中适当延长主导卵泡生长时间可以提高优质胚胎率和妊娠率。

Objective To discuss the impact of follicles growth rate domination on pregnancy outcome in the process of COH.Methods 60 patients were enrolled in mid luteal long program. The follicles ≥14mm were continuously given Gn more than 4 days in the experimental group, while the follicles ≥14mm were continuously given Gn less than 4 days in the control group. The age, duration of infertility, Gn days, the number of eggs, fertilization rate, rate of 2 pn, cleavage rate optimal embryo rate and pregnancy rate were all analyzed.Results The average age (31.57 + 2.73), duration of infertility (3.32 + 1.20 years) and the Gn days (10.21 + 1.22) showed no statistical difference between two groups (p﹥0.05). Also in two groups, the egg number was (9.50 + 1.32) and (9.27 + 1.92), the fertilization rate (82.40 + 13.13) % and (71.68 + 21.57) %, 2 pn rate (93.03 + 10.62) %and (94.91 + 10.49) % and the cleavage rate (75.61 + 12.10) % and (70.82 + 21.95) % showed no statistical signiifcance (p﹥0.05). However, the optimal embryo rate in the experimental group was (84.75 + 14.68) % ,while that in the control group was (55.45 +20.18) % and the pregnancy rate in these two groups was 50% and 20% , respectively. The difference was statistically signiifcant (P < 0.05).Conclusion In COH process, appropriate extension of the dominant follicle growth time can improve the optimal embryo rate and pregnancy rate.
