目的:本实验拟通过测定GD病人131 I治疗前后外周血辅助性T淋巴细胞膜表面CD30、CD195的表达,探讨GD病人Th1/Th2平衡机制变化及其可能的临床意义。方法:选择40例GD病人和40例健康对照组为研究对象,应用流式细胞术检测GD病人131 I治疗前及治疗后6mo的外周血淋巴细胞膜表面CD195、CD30的变化以及临床症状和甲状腺功能。结果:(1)GD病人治疗前CD195显著低于健康对照组(P<0.05);CD30高于健康对照组,CD195/CD30低于健康对照组(P<0.01)。(2)GD病人131I治疗后6mo 34例甲亢缓解。治疗后缓解组CD195上升,CD30下降,与治疗前比较有统计学意义(P<0.05);CD195/CD30上升,较治疗前变化显著(P<0.01)。结论:(1)GD的发病机理与病人外周血T淋巴细胞膜中Th 1/Th 2平衡变化机制有关。(2)GD病人131I治疗缓解组Th细胞膜表面CD195、CD30及其比值的变化有助于更好的研究GD的发生机制及对疾病短期预后的判断。

Objective:To examine the expression of CD195 and CD30 of the T-helper lymphocyte ( Th) from peripheral blood before and after 131 I therapy,in order to explore the balance mechanism of Th1/Th2 in Graves' disease ( GD ) and the clinical significance. Methods:The expression level of CD195 and CD30 of from 40 GD patients and 40 healthy controls were examined by Flow Cytometry. The level of FT 3、FT 4 and TSH were also examined by chemiluminanscent. Results:( 1 ) The expression level of CD195 of GD patients without131 I therapy was lower than that of the healthy controls (P<0. 05). While CD30 level was higher than that of controls. The ratio of CD195 and CD30 was lower than that of controls(P<0. 01). (2)34 GD patients with131I treatment showed reducedhypothyroidism. The CD195 level of GD patients showing reduced hyperthyroidism was increased and the CD30 was de-creased ( P<0 . 05 );the ration of CD195/CD30 was increased and showed significant difference comparing to the healthy controls ( P<0 . 01 ) . Conclusions:( 1 ) The mechanism of GD has the correlation with the balance of Th1/Th2 . ( 2 ) The level of CD195 and CD30 and the variation of ratio between them may be involved in pathogenesis of GD, therefore it will be useful for studying the mechanism of GD and evaluation of the short-term prognosis of 131 I therapy.
